| Human teeth is the organ for chewing function,at the same time also for pronunciation.keeping coordination and the function of speech.According to the position at the dental arch and function a person’s permanent teeth may be divided into anterior teeth and posterior teeth.Anterior teeth play a more important role than posterior teeth at beauty.So when anterior teeth missed,we shoud keep dental arch with immediate denture.The immediate denture can prevent adjacent tooth displacement,to maintain the stability of the dentition.If the remaining teeth move and tilt to the side of tooth extracted due to mechanical balance broken,it is more difficult to repair the missed tooth.At the same time,the part on the gum of immediate denture can induce gingival tissue after tooth extraction with its depth and form.So that in clinically we can change its depth and form to avoid the loss of gum original shape after tooth extraction,obtain more satisfactory result of repair.When we miss single anterior teeth,the common types of immediate denture is temporary fixed bridge and temporary elastic Denture.The first one requires the preparation of two adjacent teeth,the second requires attached to the adjacent teeth and gum nearby.With the development of dental material, the material for orthodontic retainer was also used to make an immediate denture.But which kind of immediate denture can make the gum more healthy and natural is still a question.Objective:The purpose of this clinical study was to evaluate the effect immediately using of three kinds of immediate denture after tooth-extraction on soft tissue shaping.Methods:1:Collect cases:We selected 15 patients with single hopeless maxillary anterior tooth as the research objects.2:Make immediate denture:They were divided into three groups according to three kinds of immediate denture,each group of 5 patients.Making alginate impression,making immediate denture,extracting teeth with minimal invasion for everybody.The immediate denture were placed 30 minutes after tooth extraction.Taking photos for every patient before teeth extraction and three months after teeth extraction.Visit at two weeks,four weeks,one month after teeth extraction to observe gum and inflammation,to fix the immediate denture.3:Record: Record part of PES before teeth extraction and three months after teeth extraction on the base of photos.Part of PES include:mesial papilla,distal papilla,soft-tissue level,softtissue contour,soft-tissue color and texture.Results:1: Every patient felt good about the immediate denture and was satisfied.2:All immediate dentures had well gum.3:Three kinds of immediate denture were in good function;indexs were improved.The score of mesial papilla,soft-tissue level,soft-tissue contour,soft-tissue color and texture were significantly higher(P < 0. 05).The score of distal papilla had no significant difference(P > 0.05).Three kinds of immediate denture had the same influence on part of the PES,without statistical significance(P > 0.05).Conclusion:Three kinds of immediate denture can shape the gingiva of maxillary anterior tooth well. |
