Application Of "No-touch" Isolation Technique In Surgical Treatment Of Hepatocellular Carcinoma

AIM:To study the relative indexes of patients with primary hepatocellular carcinoma who took surgical treatment using no-touch isolation technique, comparing with those who took the traditional surgical resection. And to find the advantages and disadvantages of no-touch isolation technique, explore the feasibility of no-touch isolation technique for improving the liver resection of primary hepatocellular carcinoma.METHODS:The clinical data and follow-up data of 70 cases in Anhui Provincial Hospital of Anhui Medical University from January 2011 to January 2013 using non-touch isolation technique surgical resection and pathologically confirmed to be HCC were retrospectively counted, we set it to observer group. And 70 cases of pathologically confirmed HCC patients in the same period using traditional surgical resection were set to be the control group. Some preoperative, intraoperative and postoperative observation indexes were selected for comparative analysis, including: gender, age, tumor size, preoperative AFP, whether moderate or severe cirrhosis, operation time, hepatic portal blocking rate, amount of bleeding, blood transfusion rate, complication rate, recurrence rate, survival rate, etc.RESULTS:(1) Intraoperative indicators:The operation time of observation group was longer than that of control group [(238.9+44.8) vs. min (132.8+25.4) min, P<0.05]; but the observation group was significantly lower than the control group [7.14%(5/80) vs.O 32.9% (23/70), P<0.05]; There were no significant differences in blood loss and blood transfusion rate between the two groups [(461+112.6) ml vs. (464.6+109.0) ml, P>0.05], [31.4% (22/70) vs.28.5%(20/70), P>0.05]. (2) Incidence of complications and follow-up results:There was no significant difference in the incidence of postoperative complications between the observation group and the control group [18.6%(13/70) vs.22.9%(16/70), P>0.05], the recurrence rate of the no-touch isolation technique group was significantly lower than that of the traditional group[24.3% vs.44.3%, P<0.05], there was no significant difference in survival rate between the two groups in one year[88.6% vs.81.2%, P>0.05].CONCLUSION:Comparing with the traditional operation resection, no-touch isolation technique has a certain advantage in the surgical treatment of hepatocellular carcinoma, and it may has a positive effect on reducing postoperative recurrence and improving the prognosis of patients with primary hepatocellular carcinoma.
