Analysis The Uygur Influence Factors Of Essential Hypertension With Obstructive Sleep Apnea Syndrome Patients In Heart Structure And Function

Objective:TO investigate the impairment of cardiac structure and function in patients with primary hypertension complicated by obstructive sleep apnea hypopnea syndrome and its related factors in primary hypertension patients in Xinjiang.Methods:217 hypertensives including 138 males and 79 females from the First Affiliated Hospital of Xinjiang Medical University were enrolled.According to the apnea hypopnea index(AHI),the patients were divided into 4 groups: AHI<5/h for the hypertension group(EH group, n=17),5~15/h for the hypertension associated with mild OSAHS group(mild group,n=84), and >15~30/h for hypertension with moderate OSAHS group(moderate group, n=70), AHI>30/h for hypertension with severe OSAHS group(severe group, n=45). Using echocardiography and cardiac systolic structure determination,diastolic function parameters,in addition with sleep apnea monitoring,ambulatory blood pressure determination of related parameters and compared,the data were analyzed with statistical methods to analyze the statistical significance of the indexes of the four groups and the influence factors of cardiac structure and systolic and diastolic function.Results:(1) The gender,age, smoking,drinking, BMI,fasting blood glucose,FT3,FT4,TSHwere statistically significant among the all groups(P<0.05).(2)The compared with 24 hours systolic blood pressure, 24 hours diastolic blood pressure, AHI, LSaO2, ventricular septum,posterior wall of left ventricle, left ventricular end diastolic diameters,E/A,LVEF,LVMI between the four group were also statistically significant(P<0.05).(3)The multifactor logistic regression analysis reflect that FBG(OR=2.584,95%CI=1.138-5.869),AHI(OR=2.028,95%CI=1.003-4.098),Blood pressure control(OR=3.632,95%CI=1.987-6.637)were the influence factors of LVMI;Age(OR=2.124,95%CI=1.661-2.717),AHI(OR=2.079,95%CI=1.234-3.505),blood pressure control(OR=2.739,95%CI=1.831-4.079)were the influencing factors of E/A;Sex(OR= 7.776,95%CI=2.879-21.000),Age(OR=1.562, 95%CI= 1.098-2.223), BMI(OR=1.867, 95%CI=1.377-2.530),FBG(OR=2.533,95%CI=1.035-6.200),Blood pressure control(OR= 2.797,95%CI=1.335-5.808)were the influence factors of LVEF; Conclusion: AHI is an independent risk factor for the heart structure and function damage of the uygur nationality.Uygur men are the independent risk factor for heart function.Age is an independent risk factor for the heart structure and function damage of the uygur nationality.
