| Objective:Acute traumatic coagulopathy in the multiple trauma with hemorrhagic and death plays an important role,it has recently gained increasing appreciation.But the causes and mechanisms are not fully understood.This study was conducted to assess the risk factors associated with acute traumatic coagulopathy.And the base excess(BE) was quantitative analysis. Methods:Clinical date were alalyzed retrospectively in 120 multiple trauma patients admitted to The First Affiliated Medical University.Trauma patints with an Injury Severity Score≥16 were retrospspectively analysed for independent risk factors of acute traumatic coagulopathy on arrival at the emergency trauma center by multivariate stepwise logistic regression analysis. Acute traumatic coagulopathy diagnostic criteria:prothrombin time(PT)> 18 seconds or activated partial thromboplastin time(APTT)>60 seconds or international normalized ratio(INR)> 1.5.Trauma patients were divided into coagulopathy goup and non-coagulopathy group. Results:A total of 120 patients was eligible for univariate analysis.Both groups patients with age,gender,WBC were showed no significant difference(P>0.05).ISS,shock,GCS,BE,length of stay in ICU,total hospital days,MODS,witin 24 hours of death rate,death rate, hypothermia difference was statistically significant(P<0.05).Independent risk factorts for acute traumatic coagulopathy calculated by multivariate analysis were the Injury Severity Score,base excess,hypothermia,shock. Conclusions: From multivariate analysis of risk factors in line with the current acute traumatic coagulopathy influenced by several key clinical factors.For example, the shock was associated with a increased risk of developing coagulopathy.Acute traumatic coagulopathy is higher mortality rate.The recogni,prevention,and management of the mechanisms and risk factors of coagulopathy aggravating haemorrhage after trauma are critical in the treatment of the severly injury patient. |
