Objective:By PWI study of early Alzheimer’s patients, exploring the PWI’s application value of diagnosis in early Alzheimer’s disease. Materials and Methods:Select 30 patients with early AD according to the clinical diagnostic criteria; and 15 cases of healthy controls. Adopt Siemens 1.5T Magnetom Avanto Tim + Dot super-conducted the whole body MRI scanner to conduct perfusion scan(PWI) again after scanned head plain, then introduce the perfusion images into the Siemens built-in processing workstation, finally generate pseudo-color pictures and perfusion parameters measurement including cerebral blood volume CBV, cerebral blood flow CBF, mean transit time MTT and time to peak TTP. We respectively measure bilateral frontal lobe, temporal lobe, parietal lobe, hippocampus, precuneus, parahippocampal gyrus and amygdala region, analyze sensitive indicators by Spss21.0 statistical software, then use Logistic regression to verify sensitive diagnostic perfusion index and use receiver operating characteristic(ROC) curve evaluating effectiveness. Result:Early AD group compares with the healthy group, CBV and CBF are significantly reduced, but TTP and MTT are significantly increased, which the differences among left temporal lobe r CBF, left precuneus MTT and left amygdala r CBFs were significant(p <0.05), showing early AD groups each region of interest flow time relatively longer, and decreasing tissue perfusion per unit of time. Logistic regression analysis of early AD perfusion and the left amygdala MTT, left precuneus MTT, left temporal lobe r CBF, right precuneus r CBV, left precuneus r CBV show a obvious correlation, which the effectiveness of the equation by chi-square test, X2 = 57.286, P = 0.000, is statistically significant. ROC curve analysis shows that the left temporal lobe r CBF is the largest area under the curve, p = 0.000, Cut-off value of 64.44, sensitivity of 86.667% and specificity of 93.333%.This indicates that the left temporal lobe CBF is a reliable indicator for early detection of AD.It can provide an important reference for the diagnosis of disease Conclusion:(1)The close relationship between Alzheimer’s disease and vascular factors is confirmed by PWI.(2)It proved that PWI technique to the diagnosis of early alzheimer’s disease has a guiding significance,it also can provide important help to clinical.
