Objective:To study serum 25(OH) D3 level in middle aged and elderly patients with type 2 diabetes mellitus and osteoporosis.Methods:Collect between December 2014 and December 2015, see A doctor in the hospital outpatient and the inpatient of elderly patients and the healthy check-up, according to density and whether diabetes divided into diabetic osteoporosis group(group A) 19 cases;Simple diabetes group(group B) 26 cases; 10 cases of osteoporosis group(group C); health comparison 9 cases(group D).For age, body mass index(BMI), and diabetes duration, fasting plasma glucose(FPG), glycosylated hemoglobin(Hb A1c), fasting C peptide(FCP), 25(OH) D3, alkaline phosphatase(ALP), blood calcium(Ca2 +), concentration of phosphorus(P), total cholesterol(TC), high-density lipoprotein cholesterol(HDL), low density lipoprotein cholesterol(LDL) and bone mineral density(1-4 hip and lumbar spine, femoral neck) scanning measurement, and analysis of related factors.Results:1) Osteoporosis group of women(100.0%), and diabetic osteoporosis group and the differences compared with simple diabetes group were significantly, p< 0.05;Simple diabetes group was significantly lower than pure age osteoporosis group, p < 0.05, the diabetic osteoporosis group and pure osteoporosis group were significantly higher than that of healthy control group, p < 0.05;Simple diabetes group body mass index was significantly lower than pure osteoporosis group, p < 0.05;There was no statistically significant difference between the residual group.2) The level of serum 25(OH) D3 and biochemical indexes comparison between groups: diabetic osteoporosis group, diabetes group, osteoporosis group and healthy control group of serum 25(OH) D3 average of 16.89±4.17 ng/ml, 20.21 ±4.66 ng/ml, 20.54±6.34 ng/ml, 25.60±2.64 ng/ml, Diabetic osteoporosis group were significantly lower than simple diabetes group(p< 0.05), pure osteoporosis group(p< 0.05) and healthy controls(p< 0.05);Diabetes group of 25(OH) D3 level was also significantly lower than that of healthy controls, p< 0.05;Osteoporosis group of 25(OH) D3 level was also significantly lower than that of healthy controls, p < 0.05, but diabetes and osteoporosis group compared with no significant difference, p > 0.05, no statistical significance.The proportion of each vitamin D deficiency: diabetic osteoporosis group(79%), diabetes group was 50%, osteoporosis group was 40%, health controls was 0%.3) Serum 25(OH) D3 level and other indicators of relevance: serum 25(OH) D3 level and negatively correlated with age, Hb A1c(r = 0.381, p = 0.002; r = 0.378, p = 0.011).Serum 25(OH) D3 level was positively correlated with bone mineral density(r = 0.239, p = 0.239).Serum 25(OH) D3 level and gender, BMI, course of the disease, FPG, FCP, TG, HDL, LDL and ALP, Ca2 +, P and other indicators no significant correlation(p > 0.05).4) In different bone mass as the dependent variable, in the independent variables are gender, age, BMI, course of, FPG, Hb A1 C, FCP, TC, HDL and LDL, Ca2+, P, 25(OH) D3, the above factors into logistic regression analysis, the results showed that with the increase of age, glycosylated hemoglobin, patients with type 2 diabetes, of the loss with elevated levels of 25(OH) D3, body mass index, increase bone density increased.Conclusion:1) Vitamin D deficiency is common in the middle aged elderly,and people with diabetes, osteoporosis patients, diabetic osteoporosis patients than normal people are lack of serious diabetic osteoporosis in patients with vitamin D deficiency is most serious.2) 25(OH) D3 is closely related to age, Hb Alc. Age, poor blood sugar control, 25(OH) D3 is insufficient or a lack of diabetes, osteoporosis, diabetic osteoporosis risk factors;BMI is osteoporosis protective factors.
