Objective: A study of the causes of pain after total knee arthroplasty and clinical analysis.Methods: Retrospective analysis from October 2012 to October 2012 in our hospital orthopaedic clinic for primary diagnosis income of hospital treatment for knee osteoarthritis patients in screening, conform to(1) imaging and medical examination confirmed for unilateral knee osteoarthritis Kellgren- Lawrence class Ⅲor above 2. Laboratory examination to exclude diseases such as rheumatoid disease, tuberculosis, gout, ankylosing spondylitis(3) the knee has obvious activity limitations and passive activity of up to 100 degrees or more, cases(4) no obvious internal and external double joints deformity data complete 186 patients as the research object, among which 62 were male, female 124 cases, 66 patients of knee disease, 120 cases of right knee, at age 46-82, an average of 60.3 years old.All patients in general anesthesia downlink artificial total knee arthroplasty.Observations to choose the object of study preoperative and postoperative 2 days, 15 days, 1 week, 1 month, 3 months, 6 months, 1 year replacement side knee pain and analyze reasons, and record the static and dynamic VAS score and HSS score.Results: 2 days postoperatively in patients with(1) static VAS score and 1 week after operation, 15 days, 1 month, 3 months, 6 months, 1 year static scores were significantly difference(P<0.05), the static VAS score 2 days postoperatively in patients with the overall downward trend(2) dynamic VAS score and 1 week after operation, 15 days, 1 month, 3 months, 6 months, 1 year dynamic VAS scores were significantly different(P<0.05), dynamic VAS score the overall downward trend(3) in patients with postoperative HSS score 2 days and 1 week after operation, 15 days, 1 month, 3 months, 6 months, 1 year HSS scores were significantly different(P<0.05), HSS score factors within the overall rising trend(4) joint to postoperative pain in 16 cases(8.6%), joint outside factors to postoperative pain in 7 cases(3.8%) and unknown causes to postoperative pain in 3 cases(1.6%)(5) within 1 month after TKA painful reason for acute infection, lower limb deep vein thrombosis, patellar track bad;Pain after TKA surgery march around the reasons for loose prosthesis, prosthesis stress fracture, the medial collateral ligament injury, chronic infection, lumbar disease, hip lesions.Conclusion:(1) Patients after two days of TKA in obvious pain at rest and activity after the pain, but pain significantly decrease with time.TKA postoperative pain(2)Patients with intra-articular factor(joint cavity acute infection, loose prosthesis, prosthesis peripheral stress fracture and patella trajectory, the medial collateral ligament injury, articular cavity chronic infections, etc.) is given priority to, with joint outside factors(lower extremity deep vein thrombosis, lumbar disease, hip lesions, etc.) is complementary.(3) The TKA early postoperative pain reason with acute infection, lower limb deep vein thrombus, patella trajectory is given priority to, middle-late pain reason to loose prosthesis, prosthesis peripheral stress fracture, the medial collateral ligament injury, chronic infection, lumbar disease, hip lesions.
