| Objective: Compare the difference of serum Ig A, C3 and IgA/C3 levels between the patients with IgA nephropathy and other non-IgAN, analysis of the correlation between the IgAN patients’ serum IgA, C3 and IgA/C3 levels and renal funcation index or renal pathology classification, expore the significance of complement activation in IgAN.Methods:68 cases of the renal biopsy diagnosed as primary IgAN patients and 66 cases with other primary glomerular disease were choosed in the nephrology depatement from The Third clinical medical school of The Jilin University from Novermber 2010 to December 2015, whose clinical pathological data were recorded and retrospectively recruited. 1.The above 134 patients were divided into IgAN group(68 cases) and control group(66 cases). Analysis whether there is a difference of serum IgA, C3 and IgA/C3 leves in two groups of patients. 2. According to whether the C3 is less than 0.9g/L divide the above 68 IgAN patients into: C3 reduced group and C3 normal group. Analysis the different of clinical index in two group. 3. Acroding to Lee’s classification divide the 68 IgAN patients into: group A(Lees I~II) and group B(Lees III~V), Compare the differences of two groups’ patients with serum IgA, C3, IgA/C3 level and other clinical indicators.Results: 1. IgA patients’ serum IgA, IgA/C3 were significantly higher than that of control group, diffience has statistical significance(P<0.01); But the difference of IgAN group serum C3 level with the control group had no statistical significance(P>0.05); Patients with IgAN group and the control group was no statistically significant diffience of gender and age. 2. 29 patients with reduced C3 group, 39 patients with normal group. Compared with normal serum C3 group, the reduced C3 patients’ serum ceratinine, Cystain C, 24 hours urinary protein quantitative were more hinger(P<0.05), the glomerular filtration rate and hemoglobin were lower(P<0.01). 3. Lee’s grade III-V group compared with I-II group of patients, the serum C3 level decreased significantly(P<0.01). Along with the renal pathological damage is aggravating, serum ceratinine, Cystain C, Blood Urea Niteogen level weres higer(P<0.05), glomerular fitracation rate and hemoglobin levels were lower(P<0.05).Conclusion: 1. Srum IgA, IgAN/C3 leves can be used as identification index of IgAN and non-IgAN primary glomerular disease. The loweing of serum C3 level appeared to be related with severity of renal funcation injury and pathological demage degree in patients with IgAN. IgAN patients had different degree of complement activation, complement activation associated with renal damage of IgAN patients, detecting IgAN patients’ serum IgA, C3, IgA/C3 is very significant to the diagnosis of IgA and assess the severity of renal damage. |
