Relationship Between Gestational Diabetes Mellitus Of Pregnant Women With Lipid Metabolism And Pregnant Outcome

Objective:To analyze the relationship between gestational diabetes mellitus of pregnant women with lipid metabolism and pregnant outcome. Concern about taking hypoglycemic treatment on pregnant women with GDM, at the same time pay attention to the glucose metabolism disorders with lipid metabolism disorders during pregnancy.Methods:2722 women are delivered of babies during June 1,2011 to December 31,2012, among them 464 cases were GDM patients at the first hospital affiliated Dalian medical university, while 120 GDM cases collected to monitor lipid for the study. According to the management of blood glucose, the cases were divided into well controlled group(GDM1 group) and poorly controlled group(GDM2 group),and we randomly selected 70 cases of pregnant women with normal glucose metabolism as the NGT group. We measured GDM patients’total cholesterol(TC),triglyceride (TG),high density lipoprotein(HDL) and low density lipoprotein (LDL) compared with pregnant women in the same period who is in normal glucose metabolism.And detailed the records including their age, weight, the time of childbirth,treatment and complications.Results:(1)Comparing their general messages among GDM1 group,GDM2 group and NGT group,finally, the time of childbirth was significant different P<0.05,and there were not significant difference among three other groups’massages like ages,times of pregnancy, before-pregnant BIM, and they are comparable. (2) Comparing fasting blood glucose,blood lipid and lipoprotein in pregnant women from three groups who were admitted to hospital before delivery. We found that maternal serum concentrations of TG is significantly increased in GDM1 group compared with NGT group P<0.05,the FG,TC,HDL and LDL levels in GDM1 group were not significantly different from those in NGT group;The comparison between the GDM2 group and NGT group had the same result with GDM1 group and GDM2 group,which is that FG,TC and TG were significantly different P<0.05,HDL and LDL were not significantly different. (3)Analyzed the relationship between fasting blood glucose and blood lipids levels of pregnant women which admitted to hospital before delivery,analysis indicated that there was a positive correlation between serum FG level and TG level.(4)The complication of pregnant women which from GDM1 group,GDM2 group and NGT group was compared respectively,there were not significantly different in pre-eclampsia,preterm labor,premature rupture of membrane(PROM),polyhydramnios,malformation,neonatal hypoglycemia and fetal macrosomia between GDM1 group and NGT group,while the results between GDM1 group and GDM2 group were the same.The incidence of pre-eclampsia,preterm labor,premature rupture of membrane(PROM),polyhydramnios and fetal macrosomia in GDM2 group were significantly higher than in NGT group P<0.05,but malformation and neonatal hypoglycemia were not. (5) According to the level of blood lipid in three groups were respectively divided into normal lipid level group and abnormal.GDMl group and NGT group had the same result that were in abnormal lipid group,the incidence of pre-eclampsia and fetal macrosomia increasd significantly P<0.05,but the incidence of preterm labor, premature rupture of membrane(PROM),polyhydramnios, malformation and neonatal hypoglycemia were not.In GDM2 group,there were significantly different between normal lipid level and abnormal in the incidence of pre-eclampsia,preterm labor and fetal macrosomia, while the incidence of premature rupture of membrane(PROM),polyhydramnios,malformation and neonatal hypoglycemia have no significant difference between normal lipid level and abnormal.Conclusion:The bad pregnant outcomes have relationship with lipid metabolism disorders for the GDM patients.Therefore,measuring the levels of lipid metabolism is important to the effective indicator for observing pregnant outcomes.Effectively reduction of the lipid levels can reduce the incidence of pre-eclampsia,premature birth and other adverse pregnant outcomes.
