Abstract/Summary:
BackgroundThe proportion of the number of organ demand and supply in China is 30:1, China is facing a severe shortage of donor. How to expand the source of the donor organs and reduce the mortality of the waiting period is the primary problem of organ transplantation. DCD is a Heart Beating Donation (NHBD), which is an important way for the public, the government and the international community to expand the source of organ transplantation in China. Therefore, pay attention to the organ donors, and explore the factors that affect the decision of organ donation, which can promote the development of the cause of organ donation in China.ObjectiveIn this study, we chose to collect potential DCD cases as the research object, to explore the factors that affect the heart’s death, and provide the basis for improving the system of organ donation. And to explore ways to improve the rate of organ donation in the heart.Methods1.105 cases of potential DCD cases were collected from September 2014 to October 2015, and the standard of the entry into the group was in accordance with the standard and non medical standards for the DCD potential donors.2. This study from the demographic information, coordination of the timing and environment, family members of the donation and brain death related issues of understanding, the family refused to donate the reasons, the five aspects of the interview, and detailed records of each conversation with the family. Data were collected by the potential clinical departments of the experience of the clinician to meet the basic conditions of the patients to assess the basic status of the family follow-up treatment, organ donation coordinator to coordinate the organ donation.3. To establish the database of the data input computer, using SPSS 17.0 statistical software for statistical processing. The t-test, chi-square test, Logistic regression analysis of data are processed by SPSS 17.0Results1. There are 30 cases of successful donation and 75 cases of failed donation in potential DCD patients. thus, the rate of organ loss is 71.4%, while the DCD patients have no effect on it.2. Family members of the potential DCD patients, the success rate of donation and family members of the composition, the cognition degree of the donation was related to the statistical significance (P<0.05), and the difference was not statistically significant (P>0.05).3.Among all the factors, the main reason why organ loss is from the family members, and the second reason is from the donation hospital.4.Through the multivariate logistic regression analysis, we found that "Although the potential DCD patients have signed the donation agreement during their lifetime, their family members disagree", "Be laid to rest and The whole body by the parents, not destruction based on traditional concepts", "The DCD patients are sudden death in the process of organ donation". "The attitude of tube bed doctor is "More than one thing less" and unwilling to participate in organ donation",and so, are the risk factors for organ loss.ConclusionThe traditional ideas and the understanding of the related knowledge of organ donation are the main influencing factors, which can improve the heart rate of death and organ donation. At the same time, the Department of health administration departments to increase the intensity of propaganda for organs at all levels of hospitals, the establishment of the corresponding assessment mechanism to improve the success rate of donation.
