The World Bank believes that vulnerability is the probability that an individual or family suffered some external or internal risks, to some extent it faces economic losses because these risks directly or indirectly, the economic losses associated with family income and expenditure have a direct relationship. Health risk is one of the risks that farmers may face, and health risks may cause the catastrophic health expenditures. In our rural areas, as a result of the catastrophic health expenditure, the phenomenon of "poverty caused by illness" is very serious.Although the new rural cooperative medical system started from 2003 in China, and in the past decade, NCMS participation ratios and funding criteria continues to be improved, the scope of reimbursement and the reimbursement has gradually expanded, but the impact of these measures on the farmers Medicare vulnerability is still lack of strong empirical results. Some studies consider that the NCMS routine medical services to increase utilization and lower pocket medical costs, but other aspects of the effect is not obvious, but there are also studies suggest that the implementation of the NCMS significantly reduced pocket medical expenses. Accordingly, the main goal of this study is that to analyze the health insurance system whether reduce the health burden of farmers or not.In this paper, farmer household survey data is for empirical analysis that to investigate the impact of farmers catastrophic health expenditures, the buffering effect of Medicare catastrophic health care expenditures, and health insurance for farmers vulnerability, thus providing a basis for improved farmer health insurance policy information and advice.The study includes three aspects:Firstly, analysis the incidence of catastrophic health expenditures, strength and concentration of Jiangsu Province, understand the status of catastrophic health expenditures in Jiangsu Province, and the effects of health care system to withstand the economic effects of disease risk;Second, To analyze the impact of catastrophic health care expenditures for households vulnerability of farmers, and use econometric model to analysis medical expenditures vulnerability, to determine the characteristics of farmers who need medical assistance;Finally, analysis the impact of Medicare on farmers vulnerability, understand the impact of health insurance on personal income and poverty, analysis their impact of vulnerability on farmers further.The main conclusions of the study include three aspects:1, improve the security system for farmers’ catastrophic health expendituresThe incidence of catastrophic health expenditures in different households with different income levels are different, some of the features of the vulnerability of rural households will have an impact on catastrophic health expenditures, Such as chronic diseases, elderly population and so on.2, establish a multi-level medical security systemMedicare helps to reduce the incidence of catastrophic health expenditures, including the effect of the new rural cooperative are significant, but medical aid in reducing catastrophic health expenditures is not obvious. However, the merger cannot rely on the new agricultural solely to avoid catastrophic medical expenses, but also improve medical assistance, serious illness relief, Medicare and other health insurance business model.
