Background and Purpose:Bone metastasis from esophageal cancer clinical research is less, In-depth research has not been made especially for prognostic factors, Few related research has rarely been reported in nearly 40 years,The purpose of the paper is to investigate the clinical features and prognosis of bone metastases in esophageal carcinoma patients.Methods:Retrospective analysis on March 1,1981 to December 30,2012, Chinese academy of medical sciences cancer hospital treatment and check all underwent bone scan (ECT) of 1383 cases of esophageal cancer patients was made. Meeting the screening of 131 cases of esophageal cancer patients with bone metastases were collected. All patients follow-up were made by outpatient care and telephone. Survival calculation from bone scan in the diagnosis of bone metastases to death or the time of the last follow-up. By means of the software SPSS 19.0 we did statistical analysis.Result:Incidence of bone metastases was 9.5%. Among all the 131 patients included,82(62.6%) patients had multiple bone metastases, and 49 (37.4%)patients had single bone metastases. The median time from esophageal carcinoma diagnosis to bone metastasis was 8.4 months, Thoracic vertebrae (23.4%) was the most common site, followed pelvis by (20.8%) ribs (18.2%)and lumbar vertebrae (15.6%). Incidence of spinal metastasis was significantly higher than other parts of bone metastases, Esophageal cancer among different primary site there is no statistical difference of distribution in parts of bone metastases, In newly diagnosed patients incidence of pelvic metastasis with lymph node metastasis was higher than those without lymph node metastasis, The median survival time of patients with bone metastases was 6.6months,the 1-year survival rate was 26.2%, Effective treatment of pain, therapy of bone metastases and pelvic metastasis were prognostic factors by univariate analysis (all P< 0.05). Therapy of bone metastases and pelvic metastasis, were independent prognostic factors by Cox multivariate analysis.Conclusion:Bone metastasis in esophageal carcinoma patients has a poor prognosis. Pelvic metastasis were critical factors affecting prognosis and the use of chemotherapy and radiotherapy may have a benefit for their survival.
