Objective:To evaluate the effect of phaco-chop combined stratified emulsion splitting technology in hard cataract on topical anesthesia.Methods:Retrospectively analysize 50 eyes which were diagnosed of hard cataracts in our hospital from 2013 February to 2015 February, total of 50 cases(50 eyes) were randomly divided into A and B group, each group was consisted of 25 cases. Both groups were using ultrasonic energy burst mode crystal phacoemulsification cataract but surgical methods were different. Group A was treated with Phaco-Chop combined stratified emulsion splitting technology method. Group B was treated with conventional Phaco-Chop phacoemulsification method. Comparison of the differences between two methods include effective phaco time(EPT), actual power(AP), accumulated energy complex parameter (AECP=AP×EPT). The loss of endothelial cells, Intraoperative capsular bag integrity, visual acuity and corneal edema.Results:The mean EPT of A group was (0.37±0.23)min, The mean EPT of B group was (0.35±0.17)min, the effective phaco time had no significant difference between two groups (P>0.05). The mean AP, AECP of A group was[(19.35±2.11)%, (6.67±2.44)], The mean AP, AECP of B group was[(23.54±3.22)%, (8.25±3.36)], two groups of the AP and AECP difference was statistically significant (P<0.05). In group A,23 cases achieved thorough capsular integrity, unilateral anterior capsular tear in 1 case, posterior capsular rupture in 1 case; In group B,18 cases were achieved thorough capsular integrity, unilateral anterior capsule tear in 1 case, bilateral anterior capsular tear in 1 case, posterior capsular rupture in 5 cases, there was significant difference between the two methods for the safety of capsular integrity (P<0.05). The loss of endothelial cells, Corneal transparency and uncorrected visual acuity on 1 day and lweek postoperatively between two groups had significant difference (P<0.05). Best corrected visual acuity on 1 month postoperatively between two groups had no significant difference (P>0.05).Conclusion:The method of Phaco-Chop combined stratified emulsion splitting technology for hard cataracts on topical anesthesia, it use of burst mode phaco power reduces significantly the amount of ultrasound energy. The effectively reduce complications during operation and post-operation, reduce the loss of endothelial cell, quick recovery vision, but it doesn’t prolong phacoemulsification time.
