Leave their habitual life environment and family, admitted to the department of aged homes, physiological adaptation and psychological adaptation are not small challenges.The spiritual and psychological aspects of the humanistic care needs often larger than the operation and drug treatment of. Studies have found that the elderly people of department of aged homes live comfortably, but the lack of well-being, the existence of loneliness, loss, anxiety, depression and other psychological problem. At present the foreign research bias service and attitude to family assistance, medical personnel shortage demand caused the problem,but domestic research will focus on more caused by shortage of hardware facilities of the demand mechanism. Managers focused on the construction of hardware facilities, and neglected of the elderly physiological, psychological needs.The pension business have payed great attention in the "National People’s Congress" and "CPPCC National Committee" which held in 2011, this year "NPC and CPPCC" once again put forward strengthening the construction of endowment institutions,to promote the pension business development. The nursing staff (including nurses and the nursing member)and the elderly as most contact, the humanistic care quality directly affects the quality of life of old age and department of aged homes service quality. Compared to the clinical departments, department of aged homes start humanistic care lately, and by the nursing staff education and knowledge structure limitation, there is a certain degree of difficulty of training humanistic care quality.Humanistic care quality refers to the specific cultural education through formation of a tendency to stability, internal professional temperament or character of reflected by the caring action contains the concept of humanistic care, humanistic care, humanistic caring ability of knowledge and humanistic care of perception of 4 dimensions of.Humanities quality of department of aged homes directly affect the quality of care and the elderly to nursing job satisfaction.The present domestic research regarding the quality of the humanities concern is less. Zhang Xiuwei study of nurses’ humanistic care quality structure theory model which put forward the concept of humanistic care and the dimensions of quality of construction; Liu Yuxiao studied nurse humanistic care quality evaluation tool, developed the "humanistic care nurses quality scale", which is the main tool of humanistic care quality evaluation at present; Guo Yujie to study humanities quality internalization mechanism, puts forward the general process of inner-turning, quality focus model to imitate, aesthetic, experience and comprehension, six links such as value identity, practice.At present domestic reached a consensus on the necessity of nursing care, and puts forward some training strategies. But the humanistic care quality training especially nurses from department of aged homes has not been reported. This study for the first time in the form of pension agency cooperation with colleges and universities, based on the study result of humanistic care nurses quality internalization mechanism,focus to imitate model, aesthetic, experience and feeling, value identity, practice 6 aspects,such as design to take the situation as a carrier, training programs to experience activities as the main line, understanding of humanistic care quality in the experience. In order to verify the feasibility of the design scheme of experiential training and effectiveness of nursing staff, this study selected a department of aged home in Guangzhou. Random digits table is assigned to the test group and the control group. The test group received experiential training, the control group did not intervene, then used the nurse humanistic care quality scale for training the two groups before and after the nursing staff’s humane care quality evaluation. In order for the future conduct of humanistic care quality education of nurses, improving the service quality to provide reference.OBJECTIVE1.To explore the status of humanistic care quality of nurses in department of aged home;2.To explore the influence factors of humanistic care quality of nurses in department of aged home;3.To discuss the best way of training for nurses of department of aged home to improve humanistic care quality;4.To establish an effective training plan to improve humanistic care quality of department of aged home.SIGNIFICANCE1.To explore the influence factors of humanistic care quality of department of aged home;2.To provide a new perspective for improving humanistic care quality of department of aged home;3.To enhance the humanistic care quality of nurses in department of aged home.OBJECTThe use of convenience sampling method,3 of Guangzhou city’s public pension institutions,5 private pension institutions in 270 nursing staff as the object of the first part of the investigation, including 246 nursing member,24 nurses. The second part uses the convenience sampling method to select Guangzhou city pension institutions in 100 nursing staff as the research object, including 86 nursing member,14 nurses. The SPSS software to set the random number seed the random number generator in the 20000 generation of random numbers, function and sort, before 50 people into the test group,50 were included in the control group.Inclusion criteria:(1) the nurse with the nurse practicing qualification of on-the-job nurses or to the nursing member qualification certificate; (2) the informed consent, and volunteered to participate in this study.Exclusion criteria:(1) work in the pension institutions, but have not yet obtained the nurse license or the nursing member qualification personnel; (2) during the period of nursing staff in posts, including sick leave, leave, go out to learners.METHODSThis Survey is divided into two parts.The first part of the content for research. By convenient sampling method, sampling in Guangzhou city in 8 nursing institutions 270 nursing personnel as the research object, using the method of questionnaire, the contents of the questionnaire compiled by Second Military Medical University Liu Yuxiao, consists of two parts: general information questionnaire and the nurse humanistic care quality scale. Evaluation of the research object of humane care quality, and analyze its influence factors using multivariate step-wise regression method.The second part of the content for the intervention study. Based on study of the contents of the first part, through literature reading humanistic care quality, combined with GuoYujie, Jiang Anli of humanistic care nurses quality internalization mechanism research, design to take the situation as a carrier, training programs to experience activities as the main line. In the study of nursing staff were randomly assigned to the experimental group and the control group, the implementation of 6 training courses for the experimental group nurses, training a total of 12 hours,1 times one week, completed in 2 months. While the control group did not participate in the training, still be responsible for the elderly daily nursing work.Before the first time and after the training finished, subjects were asked to fill the "humanistic care quality scale" of nursing staff. After training, the experimental group to fill the self-designed "quality of humanistic nursing care personnel of department of aged home in experiential training satisfaction questionnaire", to evaluate the feasibility of training experience and effectiveness of the scheme.STATISTICAL METHODSAll data were processed using SPSS 17. Using the mean, standard deviation, the method of descriptive statistics to describe the score than general situation, humanistic care quality scores and satisfaction of nursing staff; humanistic care quality score among the three groups by single factor analysis of variance and t test; multiple step-wise regression analysis to identify the main factors affecting the quality of humanistic care.The test group and the control group pretest and post-test differences by independent samples t test; test group and the control group before and after the training were compared using paired t test. The level of test a=0.05, P<0.05 thinks the difference had statistical significance.RESULTS1.Humanistic care quality of nurses in department of aged home is in low levelThere are 280 questionnaires, excluding the missing items and other unqualified questionnaire after 270 copies of valid questionnaires, effective recovery rate is 96.4%. The total score of humanistic care quality of nursing staff in Guangzhou city in 8 nursing institutions (113.59±9.78), lower than the evaluation of nurse in Shanghai city in 2010 part of comprehensive hospital human care quality and the scale of designer Liu Yuxiao to provide reference values (118.36±16.75), at a low level, in 4 dimensions, average scores from high to the low order as:humanistic care knowledge (4.01±0.36) points, humanistic care perception (3.9±0.45) points, humanistic caring ability of (3.86±0.43), the concept of humanistic care (3.84±0.43) points.2.Factors affecting humanistic care quality of nurses in department of aged homeAccording to the family seat, political affiliation, colleagues care and whether to accept the education and training of a packet of humanistic care, humanistic care quality score was statistically significant difference within each group (P<0.05). By multiple factors of affecting the quality of humanistic care nursing staff pension institutions of step-wise regression analysis, the main factors influencing the quality of nursing staff in this group of humanistic care for your care degree and whether they had received the humanities concern education and training.3.Effects of training plan to improve the humanistic care quality nurses in department of aged home(1) Before the first time and after the training finished,subjects were asked to fill out the "humanistic care quality scale", difference between the test group using the independent samples t test analysis after training and the control group of humanistic care quality score. The test group and the control group the results showed after the training of humanistic care quality overall score and the score of each dimension are different, and the difference was statistically significant (P<0.05).(2) By paired t test, analysis of test group and control group before and after training their differences in humanistic care quality overall score and score of each dimension. Results showed that the test group differences before and after the test, and the difference was statistically significant (P<0.05), but no significant difference before and after the test scores of the control group (P≥0.05). That experience type training to enhance the quality of nursing staff of humanistic care.(3) After training, the experimental group to fill out the self-designed "quality of humanistic nursing care personnel of old-age care institutions in experiential training satisfaction survey". Results are displayed in the 6 unit course and the teaching method, the overall satisfaction is highest is fifth unit, the life bright in communication; teaching objectives completion of satisfaction is highest is sixth unit, hospice care, let every life with dignity curtain; course content satisfaction is highest is third units, to understand the psychological needs of the elderly to life wings; the highest teaching satisfaction is the first unit, the old world need humanistic care. Scores from the situation, the nursing staff of the training scheme is satisfactory.CONCLUSIONS1.Humanistic care quality of nurses in department of aged home in low level. All the factors, the highest for the humanities equally is caring knowledge, followed by the ability of humanistic caring in the humanities concern and perception, the concept of humanistic care.2. The home location, political affiliation, colleagues care, whether to accept the education of humanistic care, are factors that influence the quality;And caring colleagues, whether to accept the education of humanistic care as main effect factors.3.The implementation of experiential training in department of aged home can effectively improve the humanistic care quality, experiential training scheme of this design is feasible and effective.
