Factors Associated With The Persistence/Recurrence Of CIN2/3 In Women Submitted To Loop Electrosurgical Excision Procedure

Objective:To give better guidance and treatment to women suffering from CIN2/3 submitted to loop electrosurgical excision procedure and to analyze risk factors associated with persistence/recurrence of CIN2/3, retrospective analysis was done on women submitted to LEEP in the Second Affiliated Hospital of Chongqing Medical University.Methods:A case-control study included 25 women with (case group) and 25 women without (control group) persistence/recurrence of CIN2/3 after LEEP from December 2012 to August 2014. Women with CIN2/3 who completed the follow-up after LEEP in our hospital are included. Women were eliminated in following conditions:women with no histopathological evidence, women with histopathologic diagnosis of Invasive cervical cancer. Datum were analyzed through SPSS19.0 statistical software:binary logistic analysis was used to evaluate the relevance between persistence/recurrence of CIN2/3 (dependent variable) and patients’ biological characteristic, social information, fertility, life-style and diseases (independent variable). Datum were analyzed with two-regression binary and multiple logistic analysis. All statistical testing were two tailed test. Odds ratio (ORs) and 95% confidence interval (95% CIs) were calculated. P<0.05 was considered statistically significant. Binary logistic analysis was used to evaluate the relevance between dependent variable and independent variables. Independent variables statistically significant analyzed in binary logistic analysis were analyzed with multivariate logistic regression analysis again.Results:1:patients amount in case group with age ≥ 40 was 4.57 times to control group (OR=4.57,95% CI=1.365-9.446, P= 0.008); In persistence /recurrence of CIN2/3 group, women from non-urban area were 4.33 times than which from urban area (OR=4.33,95% CI=1.039-6.531, P=0.014); women living in a smoking environment in case group were 3.78 times to control group (OR=3.78,95% CI=1.903-13.057, P=0.021); drinkers were 3.69 times(OR=3.69,95% CI=1.079-8.747, P=0.032); HPV persistent infection were 3.86 times (OR=3.86,95% CI=1.614-19.582, P=0.021); Women with positive cervical margins in case group were 6.47 times than control group (OR=6.47,95% CI=1.392-16.319, P=0.016); gland involved were 5.09 times (OR=5.09,95% CI=1.254-8.177, P= 0.009).2:Multivariate logistic regression analysis shows significant correlati-on between dependent variable and live in no-urban area (OR=2.48,95% CI=1.018-5.436,P=0.032); living in a smoking environment (OR=5.89, 95% CI=1.442-10.017, P=0.004); HPV persistent infection (OR=6.65, 95% CI=1.664-14.589, P=0.001); positive cervical margins (OR=4.49, 95% CI=1.115-7.012, P=0.016).Conclusions:1:The women who living in poor areas where health services, with HPV persistent infection, positive cervical margins, and living in a smoking environment are the high risk population of persistence/recurrence of CIN2/3.2:The women who are older, drinking, gland involved are more likely to persistence/recurrence of CIN2/3, and must be aware.
