Objective Early hematoma enlargement is an independent risk factor for poor prognosis in patients with intracerebral hemorrhage, this study was designed to explore the relevant factors of early hematoma enlargement, in order to provide any prediction for early clinical intervention.Methods The object of the study is from patients with supratentorial intracerebral hemorrhage treated at Department of Neurology, Neurosurgery,Critical Care Medicine, The First Affiliated Hospital of Chongqing Medical University from June 2011 to December 2014. All patients were completed brain CT scan within six hours after onset and the second brain CT scan within 24 hours interval.They were divided into two groups-hematoma enlargement group and non-hematoma enlargement group according to CT scan. We discuss the rate of early hematoma enlargement and compare clinical characteristics between the groups.Results 175 cases were included in this study,58 cases of hematoma enlargement group,117 cases of non-hematoma enlargement group. The patient’s sex, age, smoking history, drinking history, the hematoma position, admission blood pressure, blood lipids, blood glucose, blood coagulation function, high fibrinogen were no significant difference between the two groups (P> 0.05). The initial volume of hematoma, the first CT scan time,SBP>200mmHg, hyperglycemia, pulmonary infection, admitted to ICU, hospital mortality, white blood cell count were significantly different between groups (P<0.05).Conclusion The early hematoma enlargement of cerebral hemorrhage accounted for 33.1% of this study(58/175). The initial volume of hematoma, the first CT scan time, hyperglycemia, SBP≥200mmHg are the risk factors of early hematoma enlargement. Patients with early hematoma enlargement were easier to be complicated by pulmonary infection, white blood cell count increased, their condition was easier to deteriorate then admitted to ICU or died during hospitalization.
