Study On The Clinical Benefit In Patients With Dry Weight Control In Maintenance Hemodialysis

Objective:To investigate the difference of patients with maintenance hemodialysis (MHD) between dry weight controls strictly and not, and look for the benefit obtained after dry weight control strictly.Methods:We choose 98 patients from April 1,2014 and March 31, 2015, with MHD for more than 3 months in dialysis department of the second affiliated hospital of chongqing medical university. We collected the values of the dry weight by the application of body composition analyzer last month and weight of the patients after dialysis, according to the difference of the absolute value of the ratio of them, We divided patients into two group, just strict and non-strict.43 patients whose absolute ratio value was less than 0.05 were classified as strict group; 55 patients whose absolute ratio value was less than 0.05 were classified as non-strict group. We measured hemoglobin, serum calcium, serum phosphorus ion, entire section parathyroid hormone, serum creatinine of all MHD patients with stomach empty, and recorded age and duration of dialysis, the previous weight after dialysis, systolic blood pressure (SBP) and diastolic blood pressure (DBP) before every dialysis in March 2015. To observe occurrence rate of dizziness, headache, nausea and vomiting, shortness of breath, heart tired nocturnal paroxysmal dyspnea, itchy skin, poor appetite, edema in the recent 3 months, and calculated the average of SBP and DBP. According to the clinical data, compare the difference between the two groups.Results:There are 98 people meet the criteria, including 56 cases of male (57.14%),42 cases of female (42.86%),55.57±14.75 (21-85) years of age,38.96±28.16 (5-128) months of dialysis age. In the group of patients with end-stage renal disease due to Chronic glomerulonephritis, there were 47 cases (47.96%) and diabetic nephropathy in 22 cases (22.45%), hypertensive renal damage in 22 cases (22.45%), other 7 cases (7.14%). After comparing basic information, clinical symptoms, systolic pressure, diastolic blood pressure and laboratory indexes of two groups patients, results indicated mean systolic blood pressure, dialysis before the average diastolic blood pressure, the level of Hb exists significant differences(P< 0.05). The incidence rate of nausea and vomiting, shortness of breath, heart tired, paroxysmal nocturnal dyspnea, headaches, poor appetite, edema is closely related to dry weight badly control (P<0.05).Conclusions:Strictly controlling dry weight of MHD patients is beneficial to control the blood pressure, alleviate anemia and reduce the occurrence rate of heart failure and gastrointestinal symptoms.
