ObjectiveTo determine the prevalence rate of people at high risk of cardiovascular disease in Jilin Province, and to investigate the behavioral risk factors and situation of health related quality of life in the high risk population. U nderstanding the situation of high-risk types of cardiovascular disease and the aggregation of various high- risk types in subjects. Analyzing the risk factors related to cardiovascular disease in high- risk groups. Appropriate interventions were given depending on the type of risk high-risk groups, to reduce the prevalence rate of people at high risk of cardiovascular disease as far as possible, prevent cardiovascular disease and the occurrence of acute cardiovascular events effectively. MethodsThe survey was started in September 2014 to March 2015, the method of multistage stratified random cluster sampling were adopted. We selected five areas in jilin province, C hangchun C ity, Jilin C ity, Liaoyuan C ity, Yanji C ity, and Tonghua County, the screening was carried out in the community or village for the unit. A total of 100,775 people were included in the survey. Each investigator got unified training, the electronic questionnaire developed by the national center for cardiovascular disease was used, including basic information registration, screening questionnaire and high-risk questionnaire. To know the demographic information, cardiovascular health status and health-related quality of life of respondents, as well as the physical measurements(blood pressure, height and weight) and laboratory data(blood glucose, blood lipid), then uploaded the information of subjects to the national center for data platform. SPSS17.0 software was used for data analysis, health indicators and general information(such as age, height, weight, BMI, amount of cigarette smoking, blood pressure, heart rate and blood lipid) of subjects were described using mean ± standard deviation and t-test for the group comparison; enumeration data(such as demographic characteristics, behavioral risk factors, history of disease or treatment, etc.) were described using rate and chi-square test for the group comparison. The influence factors of cardiovascular disease in high-risk groups were analyzed by the unconditioned Logistic single factor and multi- factor regression method. Results(1)A total of 100,734 valid screening questionnaire were collected in this study,including 43,063 cases of male(42.7%), 57,671 female cases(57.3%). Age of subjects ranged from 40 to 85 years old, with an average age of(56.6±9.058) years old.(2)20025 cases of high risk subjects completed the questionnaire for high-risk groups. Proportion of smokers was higher in high risk groups(21.5%), the overall age of starting smoking was younger(40.4% of high- risk groups began smoking in less than 20 years old), subjects who smoke deep into the lungs were common(63.9%). Risk drinkers or alcohol dependence accounted for 10.2%. Pain/discomfort(31.3%) was the main health problems in high- risk groups of cardiovascular disease, followed by anxiety/depression(18.8%). The average score for health self- evaluation was(70 ± 15) points, the overall quality of life in general was only in the medium level.(3)The prevalence rate of people at high risk of cardiovascular disease in Jilin Province was 23.0% in 2014. We come to the conclusion that the subjects who were men, the older ones, high weight people, the non- Han and non- Korean nationality, non-agriculture account, spouse and unmarried, the higher level of education, retirement and administrative staff, annual household income in 50,000 or more had the higher rate of high risk of cardiovascular disease.(4)The prevalence rates of people at high risk in type of hypertension, cardiovascular disease, with risk greater than 20% being assessed by WHO and types for cardiovascular disease in different characteristics of subjects had significant differences(all P < 0.05).(5)Binary classification unconditioned Logistic multi- factor regression results showed that, as the increase in age, BMI, drinking frequency,TG, TC, LDL value, the risk of becoming high- risk groups for cardiovascular disease was greater. Smoking, hypertension, diabetes mellitus, unmarried, the higher level of education, and administrative staff were risk factors for cardiovascular disease in high-risk groups, women and higher HDL values were protection factors for cardiovascular disease in high-risk groups. Conclusion(1)The prevalence rate of people at high risk of cardiovascular disease in Jilin Province was 23.0% in 2014, closing to the national baseline.(2)The status of smoking and drinking in high-risk groups is not optimistic, the overall quality of life in general was only in the medium level.(3)The prevalence rates of people at high risk in type of hypertension, cardiovascular disease, with risk greater than 20% being assessed by WHO and hyperlipidemia were 15.0%, 7.8%, 5.1% and 4.8%, respectively. The prevalence rates of aggregation of various high- risk types for cardiovascular disease in subjects which having 0,1,2,≥3 kinds types were 77.0%, 15.1%, 6.2%, 1.7%, respectively.(4)The older age, higher BMI, drinking frequency, incresing of TG, TC, LDL value, smoking, hypertension, diabetes mellitus, unmarried, the higher level of education, and administrative staff were risk factors for cardiovascular disease in high- risk groups, women and higher HDL values were protection factors for cardiovascular disease in high-risk groups.
