Diffuse large B-cell lymphoma(DLBCL) is the most common non-Hodgkin lymphoma and accounts for about 30%-40% of all non-Hodgkin lymphoma. DLBCL is composed of different subtypes, and patients exhibit a wide range of clinical presentation and prognoses. Because DLBCL can happen in all different age groups, but whether there is a difference between various age groups, has not been reported. Our article is aimed at the clinical pathology research of DLBCL in different age groups and explore various clinicopathological factors and prognosis between each age groups.Objective Through the patients with diffused large B cell lymphoma(DLBCL) in different age groups, observe the clinical pathological characteristics and explore the correlation of various clinical pathologic factors between each age group. Methods The clinical data from 319 DLBCL patients who were treated in The First Hospital of Jilin University from January 2003 to May 2015 were retrospectively analyzed. Divided into 4 groups: A group: ≤44 years(67); B group: 45~59 years(111); C group:60~74 years(115); D group: ≥75years(26). Immunohistochemical staining(SP method) for CD5、CD10、CD21、CD43、Bcl-2 and Bcl-6 was performed on the paraffin-embedded tissue. The relationship between different age groups and clinicopathological characteristics was evaluated by Chi-square test.Results Through the comparison between the four age groups, pathogenic site, ECOG, Ann Arbor, IPI, CD43 were statistically significant(P<0.05). On the contrary, sex, LDH, immunophenotyping, B symptoms, Bcl-2, Bcl-6, CD5, CD10, CD3, CD21, CD30, MUM-1 were not statistically significant(P>0.05). Univariate survival analysis: number of outside involvement, LDH, B symptoms, ECOG, Ann Arbor, IPI, CD43 were related to the prognosis(P<0.05). The analysis of the prognosis in four age groups have statistic significance(P<0.05). COX multi-factor survival analysis: age, LDH, CD43 are related with prognosis closely(P<0.05). Conclusions(1) Age is an relative prognostic factor in DLBCL, the patients of ≤44 years have better prognosis.(2) The patients of ≤44 years have characteristic: gastrointestinal disease, low ECOG, low Ann Arbor, low IPI, number of outside involvement <2, Non-GCB.
