Object :To study the medium-term follow-up results in clinical spine anterior decompression and fusion procedures with expansion screw systemMethod :21 patients are selected to undergo this study.There are 14 male cases and 7 female cases in total,with the youngest at the age of 19 years old and the oldest 61 years old,thirteen of which are caused by traumatic injuries while eight by cervical disc herniation.Eight of the cases receive the single–segment decompression therapy,and the treatment for the other thirteen cases involves two segment.all the selected patients have been done the anterior cervical decompression and fusion with expansion screw system.These patients are followed up for 3 month to 2 years(mean9.7month).Preoperative,postoperative and follow-up Xray and CT are compared.in addition,the patients’ demographics, radiographic measurements, intervebral fusion rate,surgical time, blood loss,and JOA scores are included in this study.Excel is used for database, SPSS20.0 is for analysis.Result :The average surgical time is 52.48 min.The average blood loss is31.12 ml.The mean follow-up period for this study is 9.7month(range from3 month to 2 years).JOA scores(preoperative7.42±0.18,postoperative13.19±0.18,the last follow-up 14.24±0.18) have significantly raised.The cervical curvature of the patients before surgery is(15.59±1.88),and after the surery the cervical curvature is(18.74±2.32).the final follow-up data is( 16.63±1.73).The difference possesses the statistical significance(P<0.001).Compared with LSD method,the difference of the patients ’ curvature data before and after surgery has the statistical significance(P<0.001).The difference of the patients ’ curvature preoperative data and the follow-up data has the statistical significance(P<0.07). The difference of the patients ’ curvature data after surgery and the follow-up data has the statistical significance(P<0.02).The intervebral height of the patients before surgery is(6.31±1.56),and after the surery the intervebral height is(8.78±1.90).the final follow-up data is(7.59± 1.70).The difference possesses the statistical significance(P<0.001).Compared with LSD method,the difference of the patients ’ intervebral height before and after surgery has the statistical significance(P<0.001). The difference of the patients ’ intervebral height and the follow-up data has the statistical significance(P<0.07). The difference of the patients ’ intervebral height after surgery and the follow-up data has the statistical significance(P<0.01)intervebral height and radiographic measurements that both compared with the preoperative data are different(P<0.01﹚, intervebral fusion rate are 100%.until the last follow-up,internal fixtion complications don’t come up in our study.Conclusion :1. ACDF with expansion screw system,the postoperative and the last follow-up JOA scores are higher than preoperative JOA scores.The difference possesses the statistical significance.2. The cervical curvature before and after the surgery has the statistical significance and the expansion screw system keep the cervical biomechanics stability.3. It keep the intervebral herght of the cervical spine well.It is the condition for fusion.4. The follow-up after 12 month shows that all cases have the bony fusion.
