To exploring the efficiency of the technique soft release and balance of soft tissues during THA for patients with severe DDH.Methods: From October 2013 to October2015, 46 adult patients with severe DDH(50hips)were included in this study. According to the classification system, there were 26 type III and 24 type IV. Among them there were 32 women and 14 men, aged from 38 to 62 years. By thorough physical examination to the 46 cases that were going to receive total hip arthroplasty we made clear the leg length disparity and the tension of the soft tissues around the affected hip. The anteroposterior radiography of pelvis were taken. The preoperative planning was done basing on the former materials. In operation soft tissue balancing was implemented by the method recommended. After operation the differences of leg length, femoral offset, and the position of hip rotation center between the affected hip and the contralateral hip were measured on the post operative anteroposterior radiography of pelvis.Results: All patients had restoration of limb length without injury to the sciatic nerve. The follow-up ranged from 3 to 24 months(median 15 months) in 46 patients. All hips were pain free with good function at the latest follow-up.Conclusion The meticulous techniques of soft tissue release and balance can be recommended to improve abductor function during arthroplasty for DDH.
