Objective:1. To analysis the morbidity and order of the malignancy in the affiliated hospital of North Sichuan Medical College during 2001—2012 years, and to some extent understand the characteristic and trend of the malignancy in Nanchong. Contributing to formulate cancer prevention and control measures for health personnel.2. To detect the expression of HLA-G and the 14-bp ins/del gene polymorphism in adenocarcinoma of oesophagogastric junction. Then provide some evidences for adenocarcinoma of oesophagogastric junction progression and metastasis.Materials and methods:1. collected all of the malignance data from the Affiliated Hospita1 of North Sichuan Medical College during 2001 to 2012 years. Then coded and classified all cases according to ICD-10. Microsoft excel and SPSS13.0 were used to statistic the growth rate、developing rate、rank of tumors and incidence trend.2. Selected 95 adenocarcinoma of oesophagogastric junction tissues from the Affiliated Hospita1 of North Sichuan Medical College during 2012 to 2015 using as experimental group and the relevant tumor margins of 95 patients with adenocarcinoma of oesophagogastric junction were used as controls for immunohistochemistry reaction. Selected 45 cases of mucositis of esophagogastric junction tissues using as controls for PCR to detect HLA-G 14 bp ins/del polymorphism. Used SPSS13.0 statistical software for statistical analysis of the experimental data. Used descriptive statistical analysis to analyze the percentage of descrete variable; used Chi-square test to analyze the relationship between clinical parameters and immunohistochemical staining; used Logistic regression analysis to analyze HLA-G gene polymorphism and susceptibility of tumors and used Hardy-Weinberg for the distribution of genotypes. The inspection level of α <0.05.Results:1. The results of malignancy statistics: the number of malignant tumors from 2001 to 2012 years are 45411. 1972 cases in 2001 year, then increase to 5414 in 2012 year, which is 2.7 times than in 2001 year. The mean developing speed is 0.4 times. The most quick developing and increasing speed is in 2004 year. In all 45411 cases, the ratio of male and female is 1.6:1. Number of malignant tumors in male is on the rise(χ2=82.3,P=0.00). The median years both in male and female gradually increase. From 58 years up to 63 years in male, then from 55 years increase to 58 years in female. In male and female,the morbidity peak age are 60 ~ 64 years and 55 ~ 59 years, respectively. Esophageal cancer is the most common cancer both in male and female. Adenocarcinoma of oesophagogastric junction is the second and sixth cancer in male and female respectively. The percentage of digestive cancers is 70.7% in male, and 44.6% in female.2. The research results of adenocarcinoma of oesophagogastric junction: The expression of HLA-G in tumor tissues: the positive percentage of HLA-G in tumor tissues is 72.63%, but expression of HLA-G in normal tissues is no found. There is a significant relationship between HLA-G expression and invasion depth of tumor(P<0.05). ﹣14bp/﹢14bp genotype is the most both in malignant tissue and benign lesion(percentage is 54.74% and 55.56%,respectively)(P>0.05) and we found no association between 14-bp gene polymorphism and susceptibility of adenocarcinoma of oesophagogastric junction. The expression of HLA-G in patients with ﹣14bp/﹢14bp genotype is higher than the others(P<0.05).Conclusion:1. The incidence of malignant tumors is increasing year by year,and the male is more than the female. Tumors, especially in Digestive system、female reproductive system and breast are more than others in our hospital. Esophageal cancer and adenocarcinoma of oesophagogastric junction are the first and second tumors, which have local characteristcs. In summary, we should pay attention to screening and prevention of tumors.2. HLA-G is found in tumor tissue but not in normal tissue, which state HLA-G take part in development of adenocarcinoma of oesophagogastric junction. Because of a significant relationship between HLA-G expression and invasion depth of tumor, then we maybe say that HLA-G participate in tissue development and invasiveness and help to clinical treatment. The expression of HLA-G in patients with ﹣14bp/﹢14bp genotype is higher than the others and HLA-G is also related to invasion of tumor, so we take the above into account, ﹣14bp/﹢14bp genotype is the key role in genesis and development of tumors.We should detect people’s-14bp/+14bp genotype, especially in digestive tumors high-prevalence area, which could be helpful to clinic.
