From the beginning of Reform and Opening in 1978, China’s economic reform started and has beengradually deepening, along with the gradual deepening of economic reform, the health care system which is dependent on the economic system has gradually moved toward the market. However, after the reform and opening up, the health care system reform did not achieve the desired effect, difficulties and high expense in medical care became a new social problem, which has suffered the widespread complains. In order to make sure every citizen has the right to get medical treatment after illness, our government has formulated a series of medical insurance system. For example, the New Rural Cooperative Medical Insurance System, Basic Medical Insurance System for Urban Workers and Basic Medical Insurance System for Urban Residents. These systems constitute China’s basic medical insurance system. However, our health care system reform took a gradual approach.At the beginning, we designed the systemfrom part of the crowdand gradually promoted it with a strong periodicity and experimental and, inevitably, there are some limitations. It is a basic link of the medical insurance system reform process of continuous improvement that study China’s residents’ medical decision behavior and affecting facts. But all along, scholars at home and abroad who study residents’ medical decision behavior and affecting facts always put their eyes on age, gender, employment, individual income, individual education, severity of the disease, family size and so on. They ignored the medium factors like regional economic level, health care price and the number of regional medical staff. Therefore, it is necessary to consider micro-factors and regional environmental impacts on medical decision behavior comprehensively.The microdata of this paper from adult questionnaire database in 2010 of Chinese Family Panel Studies (CFPS), considering sample quantity and the require of provincial inference, this paper selects sample from Liaoning, Shanghai, Henan, Guangdong and Gansu province.By removing unsatisfactory samples and strict screening, finally, this paper got 1160 valid samples. Besides, the meso-data of this paper from China Statistical Yearbook 2010 and EPS global statistics&analysis platform.This paper bases on studying the achievement of scholars at home and abroad. According to data from adult questionnaire database and China Statistical Yearbook 2010, this paper selects agenda, age, single or not, educational level, health insurance or not, have a job or not, the serious degree of feel sick, distance from home to the nearest medical location and household per capita net income as micro-factors. Besides, urban resident per capita disposable income, every medical expenses and the number of medical treatment staff per thousand people as meso-factors. Examine their impact on medical decision-making behavior of urban residents.By fitting hierarchical linear model and running software SPSS18.0 and HLM7.0, this study finds that the medical decision-making behavior of urban residents is affected by both micro-factors and regional meso-factors. Firstly, in the micro-factors part, there is a negative influence on medical treatment rate of urban residentswhen it is low age, single, high education level, have a job, feel sick is not serious; While there is a positive influence on medical treatment rate of urban residents when it is low education level, have health insurance, feel sick is serious; Besides, gender, distance from home to the nearest medical location, household per capita net income have not influence on medical treatment rate of urban residents. Secondly, in the meso-factors part, urban resident per capita disposable income and medical treatment price level have significant influence on medical treatment rate of urban residents. The per capita disposable income of urban residents has a positive influence on medical treatment rate of urban residents. Medical treatment price level has a negative influence on medical treatment rate of urban residents. While the number of medical treatment staff per thousand people has not influence on medical treatment rate of urban residents. Besides, there are different affecting factors and same factors but different influence levels for different characteristics people. According to above conclusions, this paper put forward to five targeted recommendations. Firstly, striving to reduce unemployment rate and improve the urban employment rate, increasingurban residents’ income, narrowing the income gap between urban residents in different areascontinuously. Secondly, reducing health care cost and improving medical treatment service. Thirdly, paying more attention on the health of the young people and improving their physical fitness. Fourth, improving the education level of residents and increasing disease prevention knowledge propaganda. At last,improvingBasic Medical Insurance System for Urban Residents and Basic Medical Insurance System for Urban Workers.
