Objective: To compare the early therapeutic effect of unilateral and bilateral approach PKP in the treatment of of multiple thoracolumbar osteoporotic vertebral compression fractures.Methods: We retrospective analysed 50 patients(123 vertebral bodies) of multiple-level thoracolumbar vertebral osteoporotic vertebral compression fractures from December 2014 to January 2012,bilateral approach PKP with 25 cases(61 vertebral bodies),unilateral approach PKP with 25 cases(62 vertebral bodies).Compare the visual analogue scale,the Cobb angle, the height recovery of vertebral body, operation time, radiation exposure, bone cement usage, the leakage rate of bone cement before and after operation.Results: Operations were successfully done. The postoperative(3 days and 3months) VAS score of 2 groups was significantly lower than the preoperative score, P<0.05, which indicated that PKP by the different approaches can effectively relieve the pain of OVCF patients. The postoperative vertebral body height of all the patients was recovered, P<0.05, which indicated that PKP by the different approaches can restore vertebral height. The vertebral body height of the two groups were compared, P>0.05, which indicated that there was no significant difference between the two groups. The postoperative Cobb angle of the two groups was lower than the pretoperative Cobb angle, P<0.05. PKP by the different approach was able to correct the deformity of the spine.The recovery of Cobb angles in two groups were compared, P>0.05, which means that the recovery of Cobb values in the two groups were not significantly different. The operation times, the radiation exposures and the amount of bone cement used in the two groups were compared,P<0.05, which means that there was significantly different between the two groups. The leakage rate of bone cement in the two groups was also compared,P>0.05,so there was no different between the two groups.Conclusion: PKP by Unilateral and bilateral approach in the treatment of multiple-level thoracolumbar vertebral vertebral osteoporotic compression fractures can achieve satisfactory results. Compared with the bilateral approach, unilateral approach PKP has many advantages, such as short operation time, less radiation exposure.However, the difficulty and the requirements of unilateral approach were improved, the leakage of bone cement was not reduced.
