The Research Of Acetabular Orientation’s Determination And Accuracy In Bony Ankylosed Hip With Total Hip Arthroplasty

Objective:To explore the method of acetabular orientation’s determination in bony Ankylosed hip with artificial hip joint replacement and the accuracy of the postoperative evaluation.Methods:A continuous series of 32 consecutive patients(32 hips)accepted THA between January 2009 and March 2015 was as study group. There were 25 males and 8 females, The mean age of the patients was(35.8±12.7)years(range,18-69 years).Cause:18 cases of ankylosing spondylitis,6 cases of TB,trauma in 5 cases,osteoarthritis in 2 cases,suppurative infection in 1 case.Anteversion angle、abduction angle 、FO and AC position were measured by PACS system software in anteroposterior radiograph of the pelvis Postoperative.15°and 0 was as a reference of Anteversion angle、 Acetabulum center deviation degree to evaluate the Accuracy of them.At the same time,the abduction angle and FO,respectively,compared with the contralateral to determine its accuracy postoperative.The selected cases without the pelvis and severe lumbar spinal deformity change.Results:All of patients were complete follow-up.The average follow up time was(30.3±14.7)months(range,13-63 months).There was no statistical difference between bony ankylosed hip and the target value in Anteversion angle(P=0.630).There was no statistical difference between affected and unaffected sides in Abduction angle and FO(P=0.233,P=0.168).Acetabulum migration degree up and down average(2.1±3.6)and the internal and external migration degree average(0.4±3.2),The anatomy of the acetabulum center recovery rate was 73.6%(coincidence rate:The deviation of acetabulum prosthesis rotation center and acetabulum anatomy center with in 5mm).Conclusion:For the loss of the normal anatomy of bony Ankylosed hip,The strategy of making fulluse of intraoperative residues and permanent anatomical structure of the bony and soft tissue dissection tag structure in acetabulum positioning is more accurate ly.The rich experience of Surgeon is very important to successful operation.
