Background:At present percutaneous coronary interrention(PCI)has become effective means of treatment of coronary heart disease,it is widely accepted and used clinically because it will has invasive damage, relieve quickly symptoms of pationts and shorten the duration of hospitalization, and it also have the advantages, such as the long-term significant curative effect.This technology was started in the 1970 s, it has been 30 years of development and in development to keep improving, especially in the examination and treatment of cardiovascular diseases both made progress. Cardiovascular Interventions technical inspection, treatment programs relatively more, such as:coronary angiography, percutaneous transluminal coronary angioplasty(PTCA), coronary stent implantation technology. Every technology, every step was a direct impact on the success of the operation. Contrast-induced nephropathy(Contrast induced nephropathy, CIN) refers to the occurrence of no other reason can be explained within 48 h after the use of contrast agent cardiovascular interventional treatment of acute kidney injury. After the onset of clinical mainly serum creatinine index increased compared with the use of contrast or more than 25% increase in serum creatinine of more than 0.5mg /dl. According to the data show: contrast nephropathy mortality caused by cardiovascular intervention up to 34%, has become the hospital acquired the third largest cause of kidney damage. Currently, the clinical routine for contrast-induced nephropathy prevention, treatment method, although can improve symptoms, but poor long-term efficacy, prognosis is poor, lack of ideal treatment and care, the fact that some patients during treatment compared with treatment compliance difference. Therefore, an urgent clinical need for contrast-induced nephropathy prevention, treatment and care research,in order to reduce the incidence of contrast-induced nephropathy, the effectiveprevention and treatment. So the study of nursing intervention in the prevention of contrast-induced nephropathy after cardiovascular intervention has important significance.Refine nursing intervention is a method that based on medical science and patients’ physiological psychological and social needs,guided by humanistic care, concept by refining implementing every treatment nursing and services, to develop the corresponding nursing plan and implant it in order to improve the effect of treatment, comfort and satisfaction of nursing work from patients hospitalized until the end of the treatment.Objective: To investigate the clinical effects of nursing intervention in patients with contrast-induced nephropathy prevention of cardiovascular intervention.Methods: Methods: January 2014- December 2014 in Baoding First Central Hospital of hospitalized 80 cases of percutaneous coronary intervention and / or coronary angiography hospitalized patients met the inclusion criteria for the study were randomly divided for the control group and the experimental group, two groups of patients during hospitalization were implemented routine treatment and care; the experimental group of nursing interventions on this basis, the situation and the effectiveness of treatments were compared two groups of patients contrast-induced nephropathy occurred and verifies the effective nursing intervention and feasibility. OUTCOME MEASURES:(1) nursing satisfaction rate;(2) serum creatinine level of care before and after the index;(3) before and after treatment of blood urea nitrogen(BUN), Cr, urinary β2 microglobulin and NAG index;(4) symptom scores before and after treatment;(5) two contrast-induced nephropathy(CIN)morbidity. Statistical Methods: SPSS19.0 statistical software to create a database to improve data entry, statistical and analysis, analytical methods including independent sample T-test and chi-square test, test level: significant difference to α <0.05, significant difference was α<0.01.Results: The experimental group nursing satisfaction rate of 95 percent,significantly higher(65%); after two groups of patients before surgery care,five days after treatment serum creatinine difference was not significant(P>0.04); experimental group, two days after surgery care serum creatinine was significantly lower than the control group(P <0.05); two patient care before renal function, such as: BUN, Cr, urinary β2 microglobulin and NAG index was not significantly different(P> 0.05); experimental group nursing BUN, Cr,urinary β2 microglobulin and NAG index difference, significantly lower than the control group(P <0.05); two groups of patients before treatment symptom scores were not significant(P> 0.05); the experimental group after clinical symptom score was significantly lower than the control group(P <0.05); 3cases of contrast-induced nephropathy after treatment experimental group.Conclusion:1 Refine nursing intervention replying in cardiovascular interventional therapy in the treatment of cardiovascular diseases during the desired effect,can improve renal function in patients, improve patient treatment, quality of care satisfaction rate.2 Refine nursing intervention can improve the renal function indicators of patients who has received cardiovascular interventional therapy.3 The application of refine nursing interventions can effectively prevent cardiovascular interventional postoperative incidence of contrast-induced nephropathy, contrast-induced nephropathy patients improve awareness,understanding of the relevant preventive measures, provide a reference for clinical, reduce the incidence of contrast-induced nephropathy.4.Refine nursing intervention is safe、effective and highly practical in the prevention of CIN in cardiovascular interventional therapy in the treatment of cardiovascular diseases.
