Background:Adrenal glands are one of the most commonly involved organs in metastatic hepatocellular carcinomas. The probability of adrenal metastasis, which has worse prognosis, is 8.4% in HCC according to former autopsy-based statistical report. Since there are not typical clinical manifestation of adrenal metastasis, many patients without any symptoms were easily overlooked by physisians even patients themselves. Nowadays, the advanced imaging technology and more intensive follow-up became the main drive force for detecting metastatic adrenal hepatocellular carcinomaObject::To improve the comprehension of solitary HCC with metastatic adrenal tumor. To gain a better therapy from comparing different procedures.Method::Retrospecitvely analyzed outcome of 38 patients suffering from solitary HCC with metastatic adrenal tumor from June 2006 to June 2014 in the 1st affiliated hospital of School of Medicine, Zhejiang University.23 of 38 patients underwent surgical removal of adrenal glands (i.e. surgical treatment group, SG),3 of which had an open surgery while other 20 cases experienced laparoscopic adrenalectomy. Notably 2 paitients had bilateral adrenalectomy.9 of the included cases received non-surgical management (non-surgical treatment group, i.e. NSG), and 6 patients declined all kinds of treatment (non-treatment group, i.e. NTG).Result:All surgical removed metastatic nodules had consistent pathological results with primary HCC foci. Patients in surgical treatment group had survived 9-65 months with 29 months of median survival time, which is better than non-surgical treatment group as well as non-treatment group.Conclusion:Surgical removal of adrenal metastatic foci in solitary HCC have a benificial effect of extending the overall survival time...
