Objective:To investigate the effect of percutaneous vertebroplasty and percutaneous kyphoplasty on adjacent intervertebral disc degeneration in treating thoracolumbar compression fractures.Methods:The retrospective study involved in 49 patients with thoracolumbar osteoporotic vertebral compression fractures,which treated with percutaneous vertebroplasty or percutaneous kyphoplasty. There were 17 males and 32 females, ranging in age from 59 to 91 years.65 vertebral bodies were treated,including T8-L5.All patients were followed up for 14 to 38 months. â‘ The intervertebral discs adjacent to surgical vertebral bodies were included in the Surgery Group, and self-control discs adjacent to non-surgical vertebral bodies were included in the Control Group. â‘¡In the Surgery Group,the discs adjacent to vertebral bodies with the bone cement symmetrical distributing on both sides were included in the Symmetry Group,and the discs adjacent to vertebral bodies with the bone cement asymmetrical distributing on both sides were included in the Laterodeviation Group. â‘¢ In the Surgery Group, the discs adjacent to cartilage end plate with the bone cement touching central 1/2 area were included in the Center Group,and the discs adjacent to cartilage end plate with the bone cement untouching central 1/2 area were included in the Periphery Group. All the discs on T2W1 were classified according to the Pfirrmann grading system. Statistical analyses were conducted using SAS 8.0 software. Wilcoxon signed rank test was used for paired ordinal data. Wilcoxon rank sum test was used for qualitative data.Result:The study involved in 230 intervertebral discs. â‘ There was statistically significant difference between The Pfirrmann grade variations of Surgery Group(124 discs) and those of Control Group(106 discs)(P<0.0001),and The former is higher than the latter. â‘¡ There was no statistically significant difference between The Pfirrmann grade variations of Symmetry Group(56 discs) and those of Laterodeviation Group(68 discs)(P=0.926). â‘¢ There was statistically significant difference between The Pfirrmann grade variations of Center Group(75 discs) and those of Periphery Group(35 discs)(P=0.025),and The former is higher than the latter.Conclusion: The intervertebral disc degeneration adjacent to the surgical vertebral body can be accelerated by PVP and PKP. The degeneration is closely related to the bone cement touching central area,but the Symmetry distribution of bone cement.
