PART I PRELIMINARY EXPLORATIONS OF ANDROGEN LEVELS OF HEALTHY REPRODUCTIVE-AGE WOMEN IN CHONGQINGObjective:To explore the individual reference intervals of total testosterone(TT), free testosterone(FT), free androgen index(FAI) and dehydroepiandrosterone sulfate(DHEAS) of healthy reproductive-age women in Chongqing. Methods:Totally 121 healthy reproductive-age women were enrolled, and serum TT, DHEAS, SHBG were measured, then FT and FAI were calculated. The (x±1.96s) were calculated as the reference intervals of TT, cFT, and DHEAS, the 2.5th and 97.5th percentiles calculated as the reference intervals of FAI.Results:(1)TT, cFT and DHEAS were in normal distribution, with levels of (0.46±0.16) ng/ml, (5.92±2.80) pg/ml, (222.81±86.82) μg/dl, respectively. However, FAI was in skewed distribution with the median and interquartile spacing of 2.60%(1.87%-3.56%).(2)The 95% individual reference intervals of TT, cFT, FAI and DHEAS were (0.15~0.77) ng/ml, (0.43-11.40) pg/ml, (0.90%-9.34%), (52.64-392.98) μg/dl, respectively. Conclusion:The individual reference intervals of TT, cFT, FAI and DHEAS of healthy reproductive-age women in Chongqing are preliminarily established.PART II DIAGNOSTIC VALUE OF DIFFERENT ANDROGENS IN THE ASSESSMENT OF POLYCYSTIC OVARY SYNDROMEObjective:To explore the diagnostic value of total testosterone (TT), free testosterone (FT), free androgen index (FAI) and dehydroepiandrosterone sulfate (DHEAS) in the assessment of polycystic ovary syndrome (PCOS).Methods:117 PCOS patients and 121 age-matched healthy women were enrolled in this study. Serum TT, DHEAS and SHBG were measured, then, FT and FAI were calculated. The area under the receiver operating characteristic curve(AUC-ROC) was compared between different androgens, and cutoff values were determined.Result:(1) Compared with control group, PCOS group had significantly elevated TT, cFT and FAI values, whereas no significant differences were found in DHEAS between two groups. (2)The AUC-ROC of TT, cFT, FAI and DHEAS were 0.742±0.033,0.735±0.036,0.731±0.036, 0.589±0.037 respectively. Pairwise comparison of ROC curves showed no significant differences among TT, cFT, and FAI except DHEAS. (3)The cutoff values of TT, cFT, FAI, DHEAS were 0.63 ng/ml,7.97 pg/ml, 4.52%,229.8 μg/ml, sensitivity were 51.79%,64.29%,4.52%,54.46%, and specificity were 85.83%,85.00%,89.17%,60.83% respectively. Conclusion:The diagnostic values of TT, cFT and FAI for PCOS are equivalent, and superior to DHEAS.
