Object:Detecting the contents of Esat-6 and LAM in the cerebrospinal fluid and serum,to explore the detection application of Esat-6 and LAM for the diagnosis of tuberculous meningitis. Methods:Selecting 60 hospitalized patients in this study. All patients from First Affiliated Hospital of Shihezi University,Shihezi City People’s Hospital,Kuitun Hospital of Ili Kazak Autonomous Prefecture,Urumqi General Hospital of Lanzhou Military Region from November,2013 to August,2014.According to the clinical score of tuberculous meningitis divided into tuberculous meningitis group:20 cases, including 13 cases of possible tuberculous meningitis and 7 cases of quite possible tuberculous meningitis;non-tuberculous intracranial infection group:20 cases;control group:20cases.Record all patients’ clinical data in detail, all patients conducted lumbar puncture, conducted the examination of conventional and biochemical in the cerebrospinal fluid.Detecting the contents of Esat-6and LAM of all patients in the cerebrospinal fluid and serum by enzyme-linked immunosorbent assay, and compared.Analysis the correlation of Esat-6 and LAM in tuberculous meningitis with clinical scores, course of disease, intracranial pressure, cerebrospinal fluid cell counts,protein,glucose,chlorine.Results:(1)The examination of conventional and biochemical in the cerebrospinal fluid:lumbar puncture pressure, white blood cell counts, protein contents in tuberculous meningitis group was higher than non-tuberculous intracranial infection group and control group;glucose contents, chlorine contents in tuberculous meningitis group was less than non-tuberculous intracranial infection group and control group, the difference was statistically significant(P<0.05).(2)Esat-6 contents:The contents of Esat-6 in the cerebrospinal fluid and serum in tuberculous meningitis group was higher than that of non-tuberculous intracranial infection group and control group, the difference was statistically significant(P<0.05);the difference between non-tuberculous intracranial infection group and control group was not statistically significant(P>0.05);the contents of Esat-6 in the cerebrospinal fluid was higher than serum, the difference was statistically significiant(P<0.05);The contents of Esat-6 in quite possible tuberculous meningitis was higher than possible tuberculous meningitis, the difference was statistically significant(P<0.05).(3)LAM contents:The contents of LAM in the cerebrospinal fluid and serum in tuberculous meningitis group was higher than that of non-tuberculous intracranial infection group and control group, the difference was statistically significant(P<0.05);the difference between non-tuberculous intracranial infection group and control group was not statistically significant(P>0.05);the contents of LAM in the cerebrospinal fluid was higher than serum, the difference was statistically significiant(P<0.05);The contents of LAM in quite possible tuberculous meningitis was higher than possible tuberculous meningitis, the difference was statistically significant(P<0.05).(4)Correlation:The contents of Esat-6 and LAM in tuberculous meningitis group has positive correlation with clinical scores,CSF protein;has negative correlation with CSF chloride;has no correlation with the duration of disease, intracranial pressure,CSF cell count,CSF glucose.Esat-6 and LAM in the cerebrospinal fluid and serum was correlated in tuberculous meningitis group.Conclusion:(1)Esat-6 and LAM in the cerebrospinal fluid and serum were helpful in the diagnosis of tuberculous meningitis, cerebrospinal fluid detection was superior to serum.(2)The contents of Esat-6 and LAM may related to the disease severity of tuberculous meningitis patients.(3)Esat-6 and LAM has correlation, combined detection was conducive to the diagnosis of tuberculous meningitis.
