Objective: By using immunocytological method to detect the early secrete antigen 6 (ESAT-6) of tuberculosis bacilli in cerebrospinal fluid(CSF) mononuclear cells and to explore the method early diagnosis value for tuberculous meningitis.Methods: The 68cases of the central nervous system infection patients including the 31cases of tuberculous meningitis patients and 37 cases of not tuberculous meningitis patients were selected from neurology department of ningxia medical university affiliated hospital and neurology department and pneumology department of Ningxia fourth people's hospital during October 2008 to March 2010 . Eliminates intracranial space-occupying disease patients with cerebral hernia, suffers from, the spine disease.All selected patients were doing lumbar puncture immediately. And in a half hour for CSF conventional, biochemical, cytology dyeing inspection and alisher new lan dyed; Simultaneously cerebrospinal fluid cells producer and preservation .Then by using immunocytological method to detect early secrete ESAT-6 mycobacterial antigen and PPD antigen in CSF mononuclear cells ,while ADA detection of CSF were finished .Then the positiv rates sensitities, and specificities between every methods were compared.Results:1,The positive rate to detect ESAT-6 was significantly higher than other detection methods (P < 0.01). 2,Comparative ESAT - 6 antigen in the CSF and PB monocytes positive results , ESAT-6 in CSF specificity = 94.96%, sensitivity = 87.09%; X2 = 41.79, P = 0.00 1. Statistically significant meaning.Conclusion:1,detecting the early secrete ESAT-6 mycobacterial antigen has higher detection than detection CSF intracellular antigens PPD,ADA detection of CSF methods of specificity and sensitivity. by immunocytological method is very important for the early diagnosis of tuberculous meningitis.2,Detection antigen ESAT-6 of the sensitivit in PB is far lower than the detection directly from the CSF , So detection ESAT-6 in PB to TBM tuberculosis diagnosis meaning not sure.3,The experiment results show that TBM group,s monocytes ESAT - 6 positiv higher between the TBM group and the not TBM group, so detecting CSF monocytes antigen ESAT-6 to early diagnosis of diagnosis and differential diagnosis of TBM has certain auxiliary significance remarkable differences by application of cellular immune chemical method...
