Objective: To investigate the relationship between serum cystatin C(Cys C) level and left ventricular structure function in different causes of chronic heart failure with reduced ejection fraction(HFREF) and explore its clinical significance.Methods: Hospitalized patients with HFREF were enrolled from 2012 January to 2014 October at Department of Cardiology of Fuzhou Second Hospital as heart failure group. Patients with coronary heart disease(CHD) or hypertension and without heart failure were as control group. All patients were recorded not only general information(such as age, gender), but also liver function, kidney function, amino-terminal pro-brain natriuretic peptide precursor(NT-pro BNP), blood glucose, and lipid laboratory. Serum Cys C level and echocardiography(UCG) were performed. Serum Cys C level was compared between the two groups. The relationship of serum Cys C level with NYHA class,UCG parameters and cause of heart failure group was analyzed.Results: 1. The level of serum Cys C in HFREF group was higher than that in control group(P<0.05). Furthermore the level of serum Cys C increased with the degree of heart failure gradually(P<0.05). 2. The level of serum Cys C was negative correlation with the left ventricular ejection fraction(EF)(r=-0.426, P<0.05) but positively correlation with left ventricular end diastolic diameter( LVEDD)(r=0.567, P<0.05) and left ventricular end systolic diameter(LVESD)(r =0.323, P < 0.05) as well as amino terminal pro brain natriuretic peptide(NT-pro BNP)in HFREF group( r =0.41, P< 0.05). 3. The differences of serum Cys C level among CHD, hypertension and dilated cardiomyopathy were no statistical significance(P > 0.05).Conclusions: The contents of serum Cys C increased in HFREF, Furthermore the level of serum Cys C increased with the degree of heart failure gradually. The level of serum Cys C was positively relationship with NT-pro BNP, LVEDD and LVESD but was negatively correlation with LVEF. However, the level of serum Cys C was no obviously relationship with heart failure etiology.
