Objective Ventricular premature beat is very common in clinical. The coupling interval of homologous ventricular premature is less than or equal to 80 ms. But in the dynamic electrocardiogram, recorded coupling interval changes of homologous ventricular premature is obviously more than 80 ms.ECG scatter diagram is a major nonlinear analysis method in the field of ECG, which shows the characteristic of heart rate variability(HRV) and arrhythmia in a brand-new point of view. ECG scatter diagram will display the coupling interval changes of ventricular premature by making a diagram and show characteristics beyond what traditional ECG detection method can do. The objective of this study is to explore the lasting and variation range of coupling interval of ventricular premature by measuring of the width of plots in scatter diagram and identify the occurrence location of monophyletic premature determined by 12-channel ECG in scatter diagram reverse technology. This guides to learn the characteristics of ventricular premature scatter diagram, the occurrence location of shortest coupling interval premature beat, the relations between coupling interval changes and occurrence locations, and the links between coupling interval changes and possible mechanism.Methods With DMS12.0TOP version dynamic ECG, analyze by continuous recording of ECG signal in 24 hours and automatically make each data’s 24 h ECG scatter diagram. According to the morphological features, divide the formed ECG graphics into thin strip, rough strip, triangle, pararhythmia. Strips prove that the coupling interval is relatively fixed(divided it into the coupling interval fixation group). Rough strip and triangle prove that the coupling interval varies evidently(divided it into the coupling interval change group). The width of the long axis of scatter diagram stands for the variation range of ventricular premature coupling interval. The measured front numerical value of the width represents short coupling interval and the latter numericalvalue represents long coupling interval. To identify whether it is homologous ventricular premature beat from 12-channel ECG in scatter diagram reverse technology.Results The study results show, scatter diagram of homology of ventricular premature show thin strip, rough strip, triangle, pararhythmia four kinds of morphological features,the proportion that the coupling interval change group of homologous ventricular premature originated from left ventricular chamber is higher than the coupling interval fixation group. However, the proportion that the coupling interval fixation group of homologous ventricular premature originated from right ventricular chamber is higher than the coupling interval change group(p<0.05).The shortest coupling interval of fixation group and change group of homologous ventricular premature are 441.80±60.513 ms and 425.69±75.437ms(p>0.05) separately. The longest coupling interval for these 2 groups are 564.75 ± 74.942 ms and 708.89 ±131.652ms(p<0.001) separately. The difference of the longest and shortest coupling interval between two groups are 122.95 ±47.729 ms, 283.20 ± 118.690ms(p<0.001). The difference of coupling interval for homologous ventricular premature between two groups is over 80 ms. The occurrence cases and rates of ventricular bigeminy, trigeminy, pair and tachycardia in the coupling interval change group are higher than those in the coupling interval fixation group(P <0.05). The shortest coupling interval of monophyletic premature is originated from left and right ventricular apex. The measurements are 352 ±50.277 ms, 347.19 ± 50.845 ms, compared with the shortest coupling interval from other parts(p<0.05). The measurement of coupling interval from multiple ventricular premature originated in left and right apex is also the shortest, respectively 352.50 ± 28.158 ms, 367.73 ± 40.121 ms, compared with the shortest coupling interval from other parts(p<0.05).Conclusions 1 A high value of scatter diagram for judging the variation rang of homologous verntricular premature coupling interval. For homologous ventricular premature, when the coupling interval of electrocardiogram less than 80 ms, this judging value is not suitable for dynamic electrocardiogram.2 The scatter diagram are rough strip and triangle, it mainly has the origin in the left ventricular. However, when The scatter diagram is thin strip, it mainly has the origin in the right ventricular. 3 When the coupling interval changes greatly(The scatter diagram is rough and triangle), and more premature beat occurs in the dynamic electrocardiogram, there will be more ventricular bigeminy, pair and tachycardia to be seen. 4 ECG location of short coupling interval ventricular premature is mostly focused in left and right ventricular apex.
