【Objectives】1. To investigate the status of intravenous thrombolysis treatment process of patients with acute ischemic stroke in a grade A tertiary hospital. 2. To analyze problems existing in intravenous thrombolysis treatment process of patients with acute ischemic stroke and explore methods for optimizing the process. 3. To make a new process chart and implement optimized process.【Methods】Business process analytical method was used to optimize and improve the existing intravenous thrombolysis process for acute ischemic stroke under the theoretical guidance of process management method. 115 patients with acute ischemic stroke under intravenous thrombolysis treatment in a grade A tertiary hospital were investigated through field survey and retrospective analysis on electronic patient information. Research objects were divided into group before process optimization and group after process optimization. The group before process optimization included 37 patients admitted to hospital from January 2011 to December 2013 and the group after process optimization included 78 patients admitted to hospital from January 2014 to December 2014. DNT, NIHSS grades before and after thrombolysis, length of stay and complication of thrombolysis of research objects in both groups and satisfaction of personnel participating in this process were recorded and collected. Epidata3.0 was used for data entry and SPSS11.5 software was used for statistical analysis.【Results】1. Problems existing in thrombolysis process for acute ischemic stroke in a grade Atertiary hospital:(1) lack of rating scale available for triage by nurses;(2) lack of stroke team;(3) lack of special identification for test sample, causing the failure to complete the test as early as possible;(4) lack of green channel for emergency charging;(5) lack of drugs in reserve in emergency pharmacy. 2. Thrombolysis process optimization measures were put forward and optimized process chart was made. 3. DNT of the process after optimization shortened significantly compared to that before optimization(P<0.05). DNT was(63.62±7.97) minutes and the length of stay was(14.37±2.24) days after optimization, which had significant differences from those before optimization. Satisfaction about the process was also improved significantly(P<0.05). NIHSS grade and complication before and after process optimization did not have significant differences(P>0.05). This indicates that intravenous thrombolysis treatment is an effective therapeutic method for patients with acute ischemic stroke.【Conclusions】The application of business process optimization and management theory in hospital process management can effectively optimize thrombolysis process for acute ischemic stroke and improve the work efficiency of hospitals and satisfaction of patients and relevant workers.
