Objective:Explore the association of T2 DM body fat distribution and blood homocysteines and proteinuria, assessmenting the influence in weight loss to body fat distribution, blood homocysteine and urinary protein, the influence of early diagnosis and early intervention to provide clues for diabetic nephropathy.Methods:1. In May 2013 to October 2013 in our department in the hospital patients, to select a BMI of 28kg/m2 or obese patients with T2 DM as obese, and pure T2 DM non-obese patients as control group(DM), obesity group on the basis of the urine trace albumin/creatinine(ACR) is divided into no albuminuria group(group DM/OB) and albuminuria group(DM/OB/ACR), body fat distribution in large lab test at the same time various biochemical indicators, More related biochemical indicators between the 2groups, the differencea of measurea of body fat, and the DM/OB/ACR group ACR Hcy levels and blood, body fat distribution, correlation analysis of DM/OB/ACR group of body fat distribution index and blood Hcy levels of correlation analysis, the influence in weight loss to body fat distribution, blood homocysteine and urinary protein, the influence of early diagnosis and early intervention to provide clues for diabetic nephropathy.2. The obese patients receiing a six-month willing to accept weight loss treatment,including a lowd caloriew diets and exerciseing, six months after observation index of body fat distribution, Hcy blood, urinen protein and related biochemicalg indicators.Results:Diabetes group and diabetes/obesity group and diabetes group/fat/ACR, WC, WHR,LDL, TG, TC, Hcy, HOMA- IR, body fat distribution in large lab test at the same time various biochemical indicators, More related biochemical indicators between the 2groups, torso and thigh fat percentage, fat content than the trunk, hip fass percantage is on the riseing, both two comparison have statistical significance, DM/OB/ACR group of ACR and trunk, trunk / thigh fat percentage body fat ratio, waist/hip fat percentage,each other were very correlated, and two more are relevance, DM/OB, DM/OB/ACR group give weight loss interventions in June after treatment, two groups of WCc, WHRa and BMIc, Hcy, HOMA- Is, body fat distribution in large lab test at the same time various biochemical indicators, body fat distribution in large lab test at the same time various biochemical indicators, More related biochemical indicators between the 2groups, the DM/ACR OB/ACR group compared with the previous significantly reduced,and the DM/OB/ACR group of WC, WHR, Hcy, HOMA IR, hs- CRP by an average value is significantly higher than DM/OB group.Conclusions:1.Trunk/thigh fat percentage, runk/body fat ratio, the waist/hip fat percentage in T2 DM reflects the center of the body fat distribution, and have a good correlation to WC,WHR.2.Trunk/thigh fat percentage, runk/body fat ratio, the waist/hip fat percentage in T2 DM and plasma homocysteine and proteinuria have obvious positive correlation.3.Give weight loss intervention, the waist circumference, AGR, BMI is significantly lower, the insulin resistance is improved significantlly, plasma homocysteine is significantly lower compared with the previous, the inflammatory response is significantly reduced.Therefore, weight loss, especially central obesity, with protein resistant effect.
