Clinical Studies On Treatment Of The Centraltype Lumbar Intervertebral Disc Protrusion With Column Reconstruction Combined On Sunshang Capsules

Objective: It was to evaluate the reconstruction of posterior column of the spine in treatment of central lumbar disc herniation. The reconstruction of posterior column of was designed as treatment group, which combined use of “Sunshang capsule” of Gansu Provincial Hospital of TCM during perioperative period. The traditional total laminactomy was taken as control group. Though the comparative study between the treatment and control group in the treatment of central lumbar disc herniation, it was to investigate the clinical effect of reconstruction of posterior column in treatment of central lumbar disc herniation. The construction could keep the biomechanical stability of the spine to a large extent. It could also prevent the adverse syndrome of traditional laminectomy. The study was to provide the clinical basis of this operating type in treatment of central lumbar disc herniation. The Traditional Chinese Medicine preparation was given as drug intervention, in order to do some clinical research on Traditional Chinese Medicine combined operation in treatment of central lumbar disc herniation.Methods: Strictly in accordance with the inclusion criteria and exclusion criteria of the central lumbar disc herniation, 78 patients of central lumbar disc herniation were selected from January 2012 to January 2014 in Gansu Provincial Hospital of traditional Chinese medicine. 38 cases which used reconstruction of posterior column combined with “Sunshang capsule” were taken as treatment group, and the other 40 cases which used traditional total laminactomy were taken as control group. Lumbar JOA score and Oswestry disability index(ODI) were analyzed on preoperative and postoperative lumbar, and the recovery of the function of lower limb was also analyzed respectively. Remission of patient’s actual clinical symptoms, bone union of reconstruction and the stability of postoperative lumbar were also analyzed to evaluate the clinical effect of reconstruction of posterior column of the spine in treatment of the central lumbar disc herniation.Results: Reconstruction of posterior column had a significant superiority in relieving spinal compression and keeping the stability of the spine in the treatment of central lumbar disc herniation. The Clinical study showed, compared with the control group, reconstructionof posterior of the column of spine combined with “Sunshang capsule” in the treatment of central lumbar disc herniation has remarkable advantages in lumbar JOA score, Oswestry disability index(ODI), postoperative lumbar stability and lower limb function recovery. And there was a statistical significance.Conclusion: Reconstruction of posterior column of the spine could not only completely relieve spinal compression, but it could also shorten the operation time greatly. It fully solved the spinal stenosis, spinal epidural and nerve root compression. The damage to the overall structure of the spine was small, and internal fixation could simultaneously achieve the solid fusion of the spine.
