Objective To analyze the clinical features and therapy of bullous pemphigoid and Pemphigus, the objective of the thesis is to improve the therapy of BP and P.Methods Clinical data of 99 cases of BP and 118 cases of B from January 2005 to December 2014 in General Hospital of Ningxia Medical University、 The People’s Hospital of Ningxia 、 The First People’s Hospital of Yinchuan were retrospectively analyzed. It is illustrated in detail about the age of onset, pathogenesis, patient condition, characteristic of lesions, complication, misdiagnosis, laboratory results, therapy and prognosis.SPSS17.0 statistical software for statistical analysis, Measurement data using analysis of variance, calculators information is chi-square test,P<0.05 as statistically significant.Results In this group the rate of pemphigus in hospital of the same period was 0.90%, bullous pemphigoid was 0.76%.There are 67 mild cases, 49 moderate cases and 2 severe cases. 118 pemphigus patients, 69(58.47%) males and 49 females(41.53%)。 The ratio of male to female was 1.41:1. The average age was 49.79 ± 13.29 years old. Predilection age is 40 ~ 60 years, the number is 73(61.87%).The gender and age is basically the same for different groups. 57(48.31%) patients were farmer. The trunk was the most common onset place, and easily be misdiagnosed. Mucosal lesions occurred in 29.66%(35cases). Nikolsky 94 patients were positive,24 were negative. 42 patients associated with other diseases, hypertension 17, diabetes 8, digestive system diseases 5,other are endocrine system diseases、 immune system disease and other dermatosis. All the patients were through blood routine and biochemical test. Leukocytosis 、neutrophils、 Albumin deficiency and Dysfunction of liver were the most common phenomena. The patients were mainly treated with glucocorticoid or the combination of glucocorticoid with immunosuppressive agents, Some patients presented with mild lesion treated with glycyrrhizin. The doses of glucocorticoid were different noticeably for different degree lesions. Skin infections and lung infections were the common complication, especially infections caused by staphylococcus aureus. Among all patients, 46 cases ended with cure, 66 cases were under control, the total rate of control is 94.92%. 5 cases were unhealed, 1 cases died from pulmonary infection.There are 22 mild cases, 72 moderate cases and 5 severe cases. 99 bullous pemphigoid patients, 63(63.64%) males and 36 females( 36.36%)。 The ratio of male to female was 1.75:1. The average age was 69.99 ± 10.62 years old.Most were patients older than 60 years. About 42 cases were farmer. Trunk was the most common onset place, and easily be misdiagnosed. Mucosal lesions occurred in 2. Nikolsky 8 patients were positive. 22 patients associated with hypertension, 10 endocrine system diseases、 immune system disease and motor neurone disease, 5 other dermatosis. Laboratory abnormalities were the same as pemphigus and different noticeably for different degree lesions. The patients were mainly treated with glucocorticoid or the combination of glucocorticoid with immunosuppressive agents. Among all patients, 52 cases ended with cure,35 cases were under control, the total rate of control is 87.88%. 4 cases were unhealed.Conclusions1.The incidence of pemphigus and bullous pemphigold in Yinchuan of Ningxia had been increasing in the years between 2005 and 2014.2.These three hospitals pemphigus and bullous pemphigoid predilection for farmers groups.There was no significant difference in pemphigus and bullous pemphigold. The average age at onset of pemphigus at 57 ~ 65 years old. The age of bullous pemphigold was more than 60 years old. The trunk was the most common onset place.3.Not all patients with pemphigus Nigeria’s sign was positive, bullous pemphigoid may also occur in patients with Nigeria’s sign positive.4.Some patients in this group easily combined with other diseases, the most common diseases in the circulatory system.5.Histopathology is still an important basis for the diagnosis of pemphigus and bullous pemphigold in our region.6.Course of disease in patients prone to skin lesions of secondary infection, Staphylococcus aureus and Staphylococcus epidermidis is the most common forms.7.The initial dose of hormones, the maximum amount of control, cumulative dose, reduction of time and hospital stay largely depends on the severity of the patient’s disease. Hormone dose should emphasize the principle of individual applications.
