Objective:To observe the metabolic syndrome(MS) patients with left atrial remodeling and whether there is left atrial remodeling factors;To discuss the levels of N-terminal brain natriuretic peptide precurs(NT-pro BNP) in patients with metabolic syndrome,then to watch the relationship between the level for NT-pro BNP and the left atrial remodeling for in patients with the metabolic syndromeat the same time, to understand the possible mechanisms of left atrial remodeling;To explore the relationship between atrial arrhythmia and left atrial remodeling in patients with metabolic syndrome.Methods:To choose healthy control group of 30 people, a group of 30 people with metabolic syndrome without hypertension, metabolic syndrome and hypertension group of 30 people, a mixed group of 64 people with metabolic syndrome.Where the healthy control group,metabolic syndrome without hypertension group, the metabolic syndrome and the hypertension groups was determined by standard clinical biochemistry, triglyceride(TG), total cholesterol(TC), low-density lipoprotein cholesterol(LDL-C), high density lipoprotein cholesterol(HDL-C), fasting blood glucose(FBG), uric acid(UA), at the same time, Echocardiography was used to measure left atrial volume index(LAVI), the maximum volume of the left atrium(LAVmax) and other indicators;The other mixed metabolic syndrome group used standard biochemical assay clinical triglyceride(TG),high density lipoprotein cholesterol(HDL-C), fasting blood glucose(FBG),echocardiography was used to measure left atrial diameter index(LADI),and to measure conventional 12-lead ECG and Holter monitoring 24 h, while 12-lead ECG measurements and 24 h Holter test results according to Kleiger atrial arrhythmia classification into simple and complex set of atrial arrhythmia group.Then to compare the difference between the two groups of left atrial diameter index(LADI), maximum P wave duration(PMAX), macruze index, P wave dispersion(PWD) and Morris index abnormal frequency.To use SPSS18.0 statistical software.Measurement data was usd(x-±s)to say,mean was compared with T test and AN0VA;Dependencies between two variables used Pearson correlation analysis,Analysis of multiple variables influencing factors performed by multivariate linear regression analysis,Count data assessed by χ2test, then P <0.05 was said the difference of statistically significant.Results:(1) Compared with the healthy control group, the metabolic syndrome without hypertension group LAVI, LAVmax, LAVmix, LARV, LASV, LAVP were increased and LACV, LAEF, LVVmax not different.Compared with the healthy control group and the group of metabolic syndrome without hypertension, metabolic syndrome with hypertension disease group LAVI, LAVmax, LAVmin, LARV, LACV, LASV, LAVP, LVVmax were increased, and LAEF not different.(2) Pearson correlation analysis showed that metabolic syndrome without hypertension group and the metabolic syndrome with hypertension group LAVI was positively correlated with age, BMI, waist circumference, TG, SBP, DBP, SIL, FBG, HOME-IR, and HDL-C were negatively correlated with TC, at the same time,LDL-C, UA were no correlation.Multiple linear regression analysis showed that metabolic syndrome without hypertension group and the metabolic syndrome with hypertension group, TG, HOMR-IR were affected LAVI factors,But the largest one impacted on LAVI was TG.(3) Compared with the healthy control group, the group of metabolic syndrome without hypertension NT-pro BNP was increased; Compared with the healthy control group and the group of metabolic syndrome without hypertension, metabolic syndrome with hypertension group NT-pro BNP was further increased.(4) Pearson correlation analysis showed that the group metabolic syndrome without hypertension and the metabolic syndrome with hypertension serum NT-pro BNP was positively correlated with age, BMI, SBP, FIN, HOME-IR, UA and waist circumference, TG, TC, LDL- C, DBP, FBG were no correlation. Multiple linear regression analysis showed that the metabolic syndrome without hypertension and the metabolic syndrome with hypertension group FIN, age, SBP, BMI were factors in serum NT-pro BNP, the largest impact was FIN for serum NT-pro BNP.(5) Pearson correlation analysis showed that metabolic syndrome without hypertension group and the metabolic syndrome with hypertension group NT-pro BNP was positively correlated with LAVI, LAVmax, LAVmin, LASV, LAVP at the same time showed no correlation with LARV, LACV, LAEF.Multiple linear regression analysis showed that the metabolic syndrome without hypertension and the metabolic syndrome with hypertension group LAVI, LAVP were important factors affecting serum NT-pro BNP, where the greatest impact was LAVI.(6) In a mixed group of metabolic syndrome, compared with the simple arrhythmia, atrial arrhythmia complexity group LADI, PMAX, macruze index, PWD values were significantly increased, and Morris index were abnormalities.Conclusion:(1) Presence of left atrial remodelingwas existed in patients with metabolic syndrome,and waist circumference, TG, HOME-IR were main factors affected the left atrial remodeling of metabolic syndrome.(2)The serum NT-pro BNP was increased in patients with metabolic syndrome, and FIN, age, SBP, BMI were major factors affected the NT-pro BNP secretionin in patients with the metabolic syndrome,While NT-pro BNP in patients with metabolic syndrome was closely related to the secretion of left atrial remodeling.(3)atrial arrhythmias was aggravated after left atrial remodeling in patients with metabolic syndrome.
