| Objective: To evaluate the relationship between the characteristics of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI) parameters and the degree of pathologic differentiation,Dukes staging,T staging as well as lymph node metastasis in rectal cancer.Methods:Collected fifty six patients with rectal cancer confirmed by surgical pathology was underwent in Gansu Provincial People’s Hospital from January to October in 2014.All cases were performed by regular sequence and DCE-MRI examination,in which 36 patients were male,and 20 patients were female, aged from 40 to 83, average was 60.5±11.8.In fifty six rectal cancer patients,13 cases are well differentiated,32 cases are moderately differentiated,11 cases are poorly differentiated. 11 cases are Dukes staging A,28 cases are Dukes staging B,11 cases are Dukes staging C,6 cases are Dukes staging D.8 cases are T1 staging,8 cases are T2 staging,26 cases are T3 staging,14 cases are T4 staging.17 patients with lymph node metastasis,39 patients without lymph node metastasis.All patients were examed by SIEMENS Skyra 3.0 T superconducting magnetic resonance imaging system and 18 channel scanning phased matrix surface coil.Routine sequences consist of HASTE T2 WI coronary position,TSE T2 WI sagittal position,TSE T1 WI axis,small FOV TSE T2 WI axis,FS T2 WI axis and DWI axial.DCE-MRI,fristly scan two groups of T1- mapping sequence bofore check.Then followed by DCE-MRI scan:Adopt T1 VIBE sequence,contrast agent selects the Gd-DTPA, injection use high pressure syringe from hand vein,the rate is 3 m L/s,dose is 0.1 mmol/kg.Uninterruptibly scan 35 phase,inject contrast agent from the begin of third phase,a single phase lasts 8 s.Scan range includes the entire tumor section.After injected contrast agent, use the 20 m L saline flushing the pipe.Last underwent T1 axial, sagittal ordinary enhanced scan.Results:The Time-intensity curve(TIC) of rectal cancer and normal rectal wall around the tumor were divided into three types,in which 8 cases were belonged to the type two,48 cases were belonged to the type three.The normal rectal wall around the tumor 41 cases were belonged to the type one,15 cases were belonged to the type two.The Ktrans value increase with the worsening of the degree of differentiation,the increase of Dukes staging,T staging as well as lymph node metastasis(P<0.01).There was no statistically significant difference of Ve and Kep values.Conclusion:DCE-MRI can reflect the haemodynamics of internal tumors.TIC and Ktrans value have certain correlate with the degree of pathologic differentiation, Dukes staging,T staging as well as lymph node metastasis.TIC and Ktrans may be used in evaluation of good and malignant rectal cancer. |
