Peri-operative Dynamic Changes And Significance Of IL-6, IL-8in Patients With Thoracic Operation

Objective: To investigate the clinical significance of plasma levels of proinflammatory cytokines interleukin-6(IL-6) and interleukin-8(IL-8) in patients undergoing thoracic surgery.Methods: Plasma concentrations of IL-6 and IL-8 were measured in 28 patients during perioperative period. Blood samples were taken on the preoperative 24 h, and 1h, 2h, 3h during the operation, 8h、16h、24h、32h, 40h、48h、56h、64h、72h、120h、168h after surgery. Plasma IL-6 and IL-8 were measured by ELISA test(enzyme linked immuno-sorbent assay). The SPSS 19 software package was used for data analysis.Results:1. There was no significant difference(P> 0.05) in the levels of IL-6 and IL-8 between the experimental group before surgery concentrations and the healthy group.2. The concentration of IL-6 and IL-8 showed a rising trend from the 1st h after operation to the postoperative day 5 in the experimental group,and there was a significant statistical difference(P<0.05). The IL-6 and IL-8 reached the peak at postoperative 24h(148.47±6.57ng/l,134.81±4.91 ng/l) in experimental group,then they decreased to 118.48±6.90 ng/l and105.802±9.89 ng/l at 72 h. And the levels of IL-6 and IL-8 returned to normal levels on postoperative 168 h.Some patients’ concentrations of IL-6 and IL-8 showed double peak.3. The experimental group showed a positive correlation between interleukin-6 and Interleukin-8, where r = 0.97, P <0.05.4. The levels of IL-8 increased as IL-6, there was a clear linear relationship between them,P<0.05;The linear regression equation of IL-8 and IL-6 was: y = 17.83 + 0.976 x. The levels of IL-8 and IL-6 in experimental group were highly correlated.Conclusion:1.The value of interleukin-6 can be used to calculate the value of interleukin-8 by regression equation: value IL-8=17.83+0.976*value IL-6;2.24 h after open thoracic surgery may be the crucial period for the prevention of postoperative complications;3.24 h after open thoracic surgery, the plasma concentrations of IL-6\IL-8 continues to rise or reverse the rise again after falling( bimodal),then there should be considered the possibility of complications.
