Objective: To observe the course of decline of the salvaged red blood cells(s RBCs) after transfusing back to the patient undergoing operationMethods: 51 patients undergoing orthopedic operation were divided into two groups randomly, the intraoperative blood salvage group(Group I,n=27) and the control group(Group C,n=24).The red blood cell count, concentration of hemoglobin and free hemoglobin,the volume of blood loss during operation, the amount of postoperative urine and drainage were detected at the begin of operation, the end of operation and the 24,48,72,96 hours after operation respectively in two grongs. The venous blood of patients undergoing orthopedic operation were split,incubated by CD14 antibody and kept out of the sun,fixed and ruptured of membrane,incubated by CD235 a.And then the red blood cell count swallowed by monocyte and neutrophil were detected by flow cytometry at 24 h,48h,72 h after operation respectively in all patientsResults: The volume of blood loss of group I was significantly more than that in group C during operation, but the red blood cell count, the concentration of hemoglobin and free hemoglobin had no significant difference between two groups at end of operation. The number of patients which showed droping with concentration of hemoglobin and the count of red blood cell were more significantly in group I than that of group C at 1-3days after operation. Moreover, it was appeared in a obvious negative correlation(p=0.005)between the minimum value of hemoglobin and the volume of salvaged blood during operation in group I. however, the amount of free hemoglobin, urine and drainage had no obvious differences between two groups. The count of red blood cell swallowed by monocyte or neutrophil in group I was significantly more than that of group C at 2-3days after operation(P<0.05).Conclusion: Intraoperative blood salvage play a good protective effect to blood during operation and the s RBCs may die rapidly in 1-3 days after surgery. The reason of deathquickly of s RBCs was its been swallowed by monocyte or neutrophil.
