Objective: To investigate the clinical efficacy and safety of video-assisted thoracoscopic surgery(VATS) in treatment of elderly patients with non-small cell lung cancer(NSCLC).Methods:79 cases of elderly patients with NSCLC who underwent complete VATS lobectomy or conventional lobectomy from jan 2013 to May 2014 in our hospital were divided into two groups, which were the VATS group(n = 41), and the thoracotomy group(n = 38). Compared the two groups in Intraoperative situation(such as:operative time,blood loss, number of lymph node dissection and so on), Postoperative situation(such as:time of postoperative closed thoracic drainage time, postoperative hospitalization and so on),number of postoperative complications.Results:Both groups were performed successfully completed. Complete VATS group Operative time(170.3±52.5)min, blood loss(162.4±111.2) ml, number of lymph node dissection(18.8±2.1), time of closed thoracic drainage(7.4±2.9)d, postoperative hospitalization(10.9±4.0) d, cost of surgery(4695.9±222.8)yuan; Traditional group got operative time(162.4±39.6)min, blood loss(317.1±159.9)ml, number of lymph node dissection( 19.9±8.1), closed thoracic drainage time(9.5±2.3)d, postoperative hospitalization(13.6±5.5)d,cost of surgery(4112.1±225.5)yuan.Conclusion: For the NSCLC patients aged more than 65, grasping the adaption of operation strictly, sufficient preparation and careful preoperative evaluation are necessary for definite effect of thoracoscope treatment.And completely VATS lobectomy with minimally invasive, security,rapid recovery characteristics, but proficient skills is required to manage blood vessels and perform lymph node dissection in endoscopic. The VATS can be effective in reducing postoperative complications. |
