Objective:Video-assisted thoracic surgery(VATS)has been Widlely used in the treatment of clinical stage? non-small cell lung cancer,but the specific operation method is controversy,therefore this meta was to evaluate the clinical outcomes after VATS segmentectomy and lobectomy in patients with clinical stage? NSCLC to offer help for clinical decision ?Methods:A comprehensive search of Embase,Pubmed,the Cochrane library,CNKI and wanfang database was performed to identify studies comparing VATS segmentectomy and VATS lobectomy in treatment of clinical stage? non-small cell lung cancer,using the Text word “segmentectomy”,“lobectomy”,“lung”,“cancer or tumor or neoplasm or carcinoma or NSCLC or non-small-cell lung”,“thoracoscopy or thoracic surgery or Video-assisted or VATS or thoracoscopic”,or Chinese keywords,then we extracted literatures according to the inclusion criteria and exclusioncriteria? The data was analyzed by Revman 5.3?Result:A total of 12 retrospective control study invoving of 2058 patients were inchuded,all patients underwent radical resection of lung cancer.There were no significant differences between VATS segmentectomy and lobectomy in 1-year survival rates(OR=0.95 95%CI:0.2 1to 4.25,P=0.95>0.05),3-year survival rates(OR=1.36 95%CI : 0.61to3.04,P=0.45>0.05),5-year survival rates(OR=0.87 95%CI : 0.56 to 1.34,P=0.53>0.05),complications(OR=0.97 95%CI:0.67 to 1.39,P=0.86>0.05),total recurrences rates(OR=0.80 95%CI:0.49to1.29,P=0.35>0.05),Local recurrences rates(OR=1.02 95%CI:0.58to1.77,P=0.95>0.05);subgroup analysis stage? NSCLC tumor diameter is not more than 3cm,there were no statistical significances in all the above informations.But there was statistical significance in post-operative hospital stay both in the treatment of clinical stage?a and stage? non-small cell lung cancer.Conclusion:VATS segmentectomy in the treatment of patients with clinical stage?a or even stage? non-small cell lung cancer has good safety and effectiveness.There were no significant differences between VATS segmentectomy and lobectomy in 1-year survival rates,3-year survival rates,5-year survival rates,complications,total recurrences rates and Local recurrences rates.And VATS segmentectomy can reduce the length of post-operative hospital stay.VATSsegmentectomy can provide satifactory prognosis for stage? NSCLC and may be an alternative for the treatment of clinical stage? NSCLC.
