Full-term Efficacy Comparison With The Conventional Three-incision Thoracoscopic Esophageal Cancer Radical Surgery

Objective:To evaluate the curative effect, the whole thoracoscope and conventional three incision radical resection of esophageal cancer complications of safety and postoperative, provides the reference for the clinical treatment of esophageal carcinoma in selecting the proper operation.Methods:100 cases of esophageal cancer were selected from 2010 January to 2014 July in our hospital department of thoracic surgery during were treated as the object of study,patients with tumor stage distribution of T1 to T3, all the patients underwent esophageal resection. According to the hospital sequence coding, randomly divided into control group and observation group, 30 cases in each group, the control group underwent neck,posterolateral right chest and abdominal median three cut esophageal carcinoma resection+ lymph node dissection, observation group patients with VATS esophageal carcinoma resection and lymph nodes dissection dissection. Comparative analysis of two groups of patients in hospitalization time, amount of bleeding during the operation, the number of lymph node dissection, postoperative drainage volume, pain, gastrointestinal peristalsis,recurrent laryngeal nerve injury, complications, and lymph node of recent(3 ~ June)recurrence metastasis.Results:The observation group patients hospitalized time for(21.42 + 10.5) days, the amount of hemorrhage of(55.3+34.6) ml, postoperative drainage volume(272.4+152.3) ml were significantly less than control group corresponding to the(26.6+12.5) days,(88.36+54.1)ml, surgery after drainage(365.4+152.2) ml, the difference was statistically significant(P <0.05); patients in the observation group and the control group patients compared to the number of resected lymph nodes, there was no significant difference(P>0.05); pain,postoperative recurrent laryngeal nerve injury patients in observation group weresignificantly lower than the control group, gastrointestinal tract peristalsis recovery was significantly higher than the control group, the difference was statistically significant(P<0.05); no significant difference between the two groups of patients with complications, no statistical significance(P>0.05); observation group lymph node metastasis rate was 40%,and 44.0% in the control group, the difference was statistically significant(P > 0.05).Conclusion:thoracoscopic resection of esophageal carcinoma and three with conventional incision in radical resection of esophageal cancer compared with operation, shorter hospital stay, less bleeding, obvious curative effect, high safety, has high clinical application value.
