Construction And Primary Application Of Clinical Database Of Thyroid Surgery

Purpose1、Create the database system which is used for clinical researches of thyroid surgery.Use this system to save and check、analyze and summarize、communicate and share the clinical information.2、Collect the whole clinical information of patients who underwent the endoscopic thyroidectomy via transoral-vestibular approach, and study its clinical application value.Method1、Collect the clinical information of 2164 patients with thyroid diseases who were treated in the first affiliated hospital of Jinan University from January 2011 to December 2014. Input those information into the database system,and divide it into groups according to ages、genders、surgical procedures and categories of diseases, and make use of SPSS13.0 for statistical analysis.2、The clinical information of patients who underwent the endoscopic thyroidectomy via transoral-vestibular approach was selected and extracted from the database.Analyze the safety、feasibility and cosmetic results of this kind of procedure to study its value.Result1 、 From 2011 to 2014,the number of inpatients with thyroid diseases in surgery increased year by year. In total of 2164 patients were received and underwent 1903 surgical operations in total during the 4 years.Male: female is 1:3.87. The age of onset was concentrated to 30~60 years old.The number of inpatients with nodular goiter was the most,accounting for 69.6%. The constituent ratios of all kinds of thyroid diseases fluctuated year by year,and hyperthyroidism showed a trend of decrease and the number of patients who were diagnosed with thyroid cancer or underwent thefollow-up treatments caused by thyroid cancer had a trend of increase.2、In total of 57 cases underwent the endoscopic thyroidectomy via transoral-vestibular approach, and all the operations were completed successfully without any conversion to open operations. The average operation time was 107.3minutes(range,60~205minutes),the average operation blood loss was 18ml(range,5-30ml),the average length of post-operative hospital stay was 5.2days(range,3~11d), fevers occurred in 4 cases、inflammation in 2 cases、small pieces of bruises on 4 patients’ chin skin、small laceration on 1 case’s jaw skin after their operations.No postoperative bleeding or inflammation,no permanent damages to recurrent laryngeal nerves、superior laryngeal nerve and parathyroid glands. All the patients were satisfied with the cosmetic results.Conclusion1、The clinical database management system for patients who were treated in thyroid surgery is able to provide enough case information for clinical and fundamental researches in the field of thyroid surgery.2 、 The endoscopic thyroidectomy via transoral-vestibular approach has a clinical application prospect for its safety、feasibility and wonderful cosmetic results.
