Background:Colorectal cancer is a common digestive tract malignant tumor, In China, there is an upward trend in its morbidity and mortality in recent years, has become the fifth most common malignancy in our country. Because of its early symptoms is not typical, when doctor has middle-late, prognosis is poorer.The intestines of colon polyps is a common pathological changes, and closely related to the occurrence of colorectal cancer, the polyp of canceration rate1.4%- 20.4% in China. The adenomatous polyp as precancerous lesions has been widely recognized, And most of the large intestine cancer is by colon polyp progression, colorectal cancer merger colon polyps are more see.In recent years, more and more people in the region a colonoscopy, intestinal polyps and detection of colorectal cancer and other diseases have increased, but so far, the transient value of intestinal polyps in the region and the analysis of the clinical pathological characteristics of colorectal cancer research reports.Objective:To preliminary understanding yan’an colorectal cancer and colorectal cancer merger cases of intestinal polyps and general epidemiological characteristics, to explore the characteristics of colon polyp canceration, for colorectal cancer screening and early diagnosis and polyp canceration process block provides scientific basis.Methods:Through the analysis of yan’an university hospital stomach and room between January 2004 and January 2004 colonoscopy database data, excluding follow-up cases, the patients’ personal information and clinical data, including age, gender, urban and rural sources, tumor, or(and) the part of the polyp, pathological type, etc. Will receive the above data by the method of descriptive epidemiology, survey population characteristics of patients with colorectal cancer, urban and rural distribution and the characteristics of the change over time and colorectal cancer merger intestinal polyps in different gender, age, polyp pathological types and multiple or single polyp detection. Colorectal cancer:(1) Characteristics: according to the age to be divided into young group(﹤40), middle-aged group(40 ~ 60 years old), elderly group(﹥60 years old).Pathological types in patients with different gender there was no statistically significant difference between the composition(P﹤0.05), but more men than women detection of cases are in each group; Are in different age groups to adenocarcinoma but mucous adenocarcinoma and signet ring cell carcinoma in young group, elderly group in a clear up, the difference was statistically significant between age pathologic types(P ﹤0.05).Between groups, both in colorectal cancer good hair, left half colon followed, right-side colon minimum, and the difference was statistically significant between different age groups. The composition of different gender in tumor patients good hair parts there was no significant difference(P﹥0.05)(2) Distribution between urban and rural areas: according to the patients with the source of urban and rural divided into urban and rural group, two groups in the pathological types and tumor location differences had no statistical significance(P﹥0.05).The pathological type was adenocarcinoma, good location in the rectum, left half colon times, less right-side colon.(3) S change: by the time the data can be divided into A, B two groups, namely the group A for five years before, after the group B for five years, A, B two groups in the composition of tumor pathological type difference has no statistical significance(P﹥0.05).Good tumor location in the two groups are in the rectum, but group B in the right half colon cancer, rectal cancer declined accordingly, the difference was statistically significant(P﹤0.05).A,B two groups of men and women ratio difference decreases, and detection rate of youth is reduced, the elderly group in the corresponding detection rate slightly increased. Different gender, age, sources of urban and rural areas in A, B two groups has no statistical significance of difference in the detection rate(P﹥0.05). 3. Colorectal polyps merger of colorectal cancer(1) The situation of three groups of detection rate: 15347 cases with an endoscopic inspectors, pure polyps in 1948 cases, 12.69% detection rate; Colorectal polyps merger of colorectal cancer in 385 cases, 2.50% detection rate; Pure, 153 cases of colorectal cancer, detection rate of 1.00%.Visible detection rate is different between the three groups, each group two, significant differences(P﹤0.05).(2) Gender detected between the three groups: pure polyps group of male, female detection rate difference was statistically significant(P﹤0.05), different gender in colorectal polyps merger of colorectal cancer, pure colorectal cancer detection rate difference has no statistical significance(P﹥0.05).(3)Different ages colon polyps, comparing the status of the colorectal polyps merger of colorectal cancer detection: with the increase of age, the detection rate were increased in both groups, statistically significant differences(P﹤0.05).(4) single or multiple polyps with colorectal cancer: patients with colorectal polyps combined colorectal cancer are less combined single polyp was much higher, difference was statistically significant(P﹤0.05).(5)The pathological type of polyps merger colorectal cancer: merge colorectal polyps in patients with colorectal cancer and adenomatous polyp detection rate is higher than the adenomatous polyp, difference was statistically significant(P﹤0.05).Conclusion:1. Colorectal cancer, colon polyps detection rate increasing year by year; 2. The characteristics of colorectal cancer(1) The crowd: men detection rate than women, the differences between men and women in recent years gradually decreased; In different age groups still, elderly group detection rate is higher; City detection rate slightly higher than the countryside, gap is narrowing.(2) The tumor location: in the rectum, change over time, good tumor location a tendency to moves to the right;(3) Tumor pathological type: with adenocarcinoma, change over time, proportion of adenocarcinoma increased, mucinous carcinoma increased slightly, and signet ring cell carcinoma was down. Youth proportion of mucinous carcinoma and signet ring cell carcinoma is higher, the elderly group is given priority to with adenocarcinoma;3. Colorectal polyps merger of colorectal cancer: old age, male patients with the highest detection rate, than adenomatous polyp atypical adenomatous polyp easier combination of colorectal cancer. Multiple polyps than single polyps are more likely to merge colorectal cancer.
