Objective: To analyze the correlation of collateral circulation in patients with unilateral carotid artery severe stenosis or occlusion, and to discuss its relationship with ischemic stroke.Methods:150 patients with unilateral carotid artery severe stenosis or occlusion on DSA, assess the opened collateral circulation, evaluate the NIHSS score on admission and two weeks after treatment, and analysis its relationship with nerve function. Results: The numbers of opened collateral circulation observed is 124 in 150 patients with unilateral carotid artery severe stenosis or occlusion. The numbers of primary collateral circulation observed is 74, among which: ACo A is 19, PCo A is 20, both ACo A and PCo A is 35. 50 patients with secondary collateral circulation, of which: leptomeningeal anastomosis is 29, ophthalmic artery is 21. The others is 26, belongs to the without collateral circulation. The NIHSS scores in patients with or without collateral circulation were significant differences(P<0.05). There were statistical differences of the NIHSS scores at discharge(P<0.05), the patients with primary and secondary collateral circulation. Conclusion: The prognosis of patients with unilateral carotid artery severe stenosis or occlusion could be predicted by the formation of the collateral circulation, which could be assessed by DSA. The stroke and its type are associated with the integrity of collateral pathways.
