| Objective To compare the effects of propofol and sevoflurane anesthesia on inflamema-tory cytokines, Oxidation-antioxidant balance and dynamic compliance during perioperative period in patients undergoing one-lung ventilation. To investigate the protective effects of pro-pofol and sevoflurane on the lung injury during one lung ventilation. To provide theoretical basis for reasonable clinical medication.Methods Forty ASAâ… orâ…¡patients(24 male, 16 female), aged 45-70 yr underwent elec-tive pulmonary resection, were randomly divided into two groups: group P(propofol group n=20), group S(sevoflurane group n=20). Anesthesia induction is practiced in two groups by intravenous injection: Midazolam0.05mg·kg-1, sufentanil 0.3-0.5ug·kg-1, etomidate 0.15-0.3mg·kg-1, Cisatracurium Besilate0.15mg·kg-1. Anesthesia maintain group P: continuous intra-venous Propofol infusion 4-10mg·kg-1·h-1 with miero-Pumpa; Group S: inhale l%-3%sevoflurane. Systolic blood pressure(SBP), Diastolic blood pressure(DBP), Mean arterial pressure(MAP)and Heart rate(HR) were recorded at before induction of anesthesia(T0), before the start of one-lung ventilation(T1), 30 min after one-lung ventilation(T2), and at the end of the operation(T3). At T1, T2, T3, record Tidal volume(VT), Peak airway pressure(Ppeak), airway mean pressure(Pmean), Positive end expiratory pressure(PEEP), calculate dynamic comp-liance(Cldyn). At T0, T1, T2, T3 aterial blood were taken for ElISA to measure Interleukin-6-(IL-6), Interleukin-10(IL-10), Malondia dehyde(MDA), Superoxide Dismutase(SOD)concentration.Results Compared with T0, atT1, T2, T3, groupP and S of SBP, DBP, MAP, HR decree-sed significantly(P< 0.05); comparison between groups, with no significant difference between two groups(P> 0.05). Compared with T1, atT2 two groups of VT decreased significantly(P<0.05), at T2, T3 two groups of Ppeak, Pmean increased, Cdyn decreased significantly(P<0.05); comparison between groups, with no significant difference between two groups(P>0.05). Compared with T0, the concentrations of IL-6 ã€IL-10 〠MDA in both groups were significantly increased at T2, T3(P<0.05),While the expression of SOD in both groups were decreased significantly(P<0.05); Compared between the groups, compared with group P, the concentrations of IL-10, SOD in group S were significantly decreased at T2, T3(P<0.05), the concentrations of IL-6, MDA In group S were increased significantly at T2, T3(P< 0.05).Conclusion Compared sevoflurane anesthesia, Propofol anesthesia was more conducive to maintaining with pro-inflammatory cytokines and anti-inflammatory cytokines balance,oxidation and antioxidant balance in patients undergoing one-lung ventilation, It may protect the lung from the injury during one-lung ventilation, And the blood dynamics and dynamic compliance has no obvious influence, Was more suitable for clinical medication. |
