Objective To reconstruct the three-dimensional image of normal liver using Philips Brilliance i CT(itelligent CT) 256, extracting the tri-dimensional model,measuring volumes of each segment to provide reference for the options of clinical treatment.Methods Collected 48 healthy patients’ data who were treated by the Health Management Center,undergoing liver enhancement with the Philips Brilliance i CT256, the images of portal venous phase will be transmitted to the specialized software(Liver Segmentation) in EBW4.5 to reconstruct the three-dimensional images of livers. Measuring the total liver volume, drawing a scatter plot between body surface area and liver volume, to obtain the linear regression equation.Finally, according to Couinaud classification, dividing each segment ofⅠ-Ⅷ, and measuring the volume of each segment.Results The mean total liver volume was(1343.2 ± 238.3) m L for 48 cases,which was positively correlated with body surface area, the correlation coefficient was 0.87, the regression equation: LV(m L) =763.0×BSA-28.6. SegmentⅠ(24.5 ±4.3) m L, segmentⅡ(148.6 ± 31.6) m L, segmentⅢ(110.4 ± 24.9) m L,segmentⅣ(222.3 ± 43.0) m L, segmentⅤ(212.8 ± 36.5) m L, segmentⅥ(186.7 ±34.7) m L,segmentⅦ(164.3 ± 30.8) m L, segmentⅧ(273.4 ± 56.1) m L respectively. The percentages of segmentⅠ- Ⅷ were(1.8 ± 0.2)%,(11.0 ± 1.1)%,(8.2 ± 1.1)%,(16.6 ± 1.4)%,(15.9 ± 1.0)%,(13.0 ± 1.0)%,(12.2 ± 0.7)%,(20.3 ± 1.5)%respectively.Conclusion The three-dimensional images of livers and hepatic segments can be well reconstructed by the Liver Segmentation with the i CT 256. The data of hepatic segment volumes can provide reference for the living donor liver transplantation, precise hepatectomy and the assessment of liver function.Objective To measure the volume of each segment in cirrhosis using256-slice i CT, analysis the correlation between liver volume and liver reserve function, to investigate the quantitative variation of each segment’s volume with the severity of liver function, and provide guidance for the clinical accurately assessment of liver function.Methods Collected 52 patients’ Clinical data who were diagnosed with cirrhosis, calculating the MELD score and Child score respectively. Measuring the total liver volume using i CT, obtained the standardization of the total liver volume(SLV), comparing the correlation between SLV and MELD score, Child score.52 patients with cirrhosis were divided into three groups according to MELD scroe including Group1(MELD score ≤10) in 18, Group2(11≤MELD score ≤20) in 20,Group3(MELD score ≥21) in 14,and 48 subjects with normal liver were regarded as a control group. Finally, according to Couinaud classification, measuring the volume of each segment to study the proportion of each section, Each segment’s Volume variation between different groups were compared and analyzed.Results There were negative correlation between SLV and Child score,MELD score, the correlation coefficients were RChild =-0.685(P <0.05),RMELD=-0.82( P< 0.05) respectively, the coefficient of determination were0.47,0.67 respectively. The absolute value of segmentⅠ’s volume was increased in Group1、Group2, after that it decreased, but the percentage was always increased relatively. The absolute value of left lateral’s volume which including segment Ⅱand Ⅲ was increased in Group1, after that they decreased, but the percentage were always increased relatively. The volume of left inner lobe(segment Ⅳ) was decreased with the change of MELD score, but the percentage of it was not statistically significant difference. There were no statistically significant difference either the volume or the percentage of segmentⅤ、Ⅵ were decreased in Group1. The right lobe’s volume decreased was statistically significant difference with the change of MELD score.Conclusion The cirrhosis has better correlation between SLV and the MELD score; Hepatitis B-related cirrhosis of the liver volume is asymmetrically atrophied. We can not only observe the change of overall morphological, but also measure volume of each segment accurately and study the quantitative variation in cirrhosis with the i CT 256. The liver reserve function can be evaluated more accurately. There have important clinical significances that we can understand the clinical disease progression accurately, adjust treatment plan and judge the prognosis.
