| Objective: To compare the treatment effectiveness of Open the vertebral canal bone graft and internal fixation operation and the pedicle of vertebral arch bone graft by minimally invasive internal fixation operation in the treatment of thoracic and lumbar fractures, provides scientific basis for clinical application.Methods: Collecting the clinical data of 39 cases who were diagnosed to be thoracic and lumbar fractures in Affiliated Hospital of Zunyi Medical College from March 2013 to May 2014 and contrasting the effectiveness of the two different surgical approaches.:1. Open the unilateral vertebral canal bone graft and internal fixation operation;2. the pedicle of vertebral arch bone graft by minimally invasive internal fixation operation;Minimally invasive operation 16 cases,open operation 23 cases,Postoperative follow-up for 8~15 months.Basing on two patients` the condition of related indexes of operation period, follow-up imaging indexes, the improvement of postoperative back pain symptoms,we undergo a comparison and make a statistical analysis.Results:1ã€Perioperative period parametersthe minimally invasive group in the operation time,the intraoperative blood loss, postoperativeoperation area drainage volume and length of hospital stay have the advantages over the control group,but the X-ray exposed for a long time, Compared with these parameters,minimally invasive group and open operation were significantly different (P<0.05).2ã€The results of imagingThe postoperative anterior vertebral height, Cobb angle, sagittal index of minimally invasive group and open group are obviously improved compared with preoperative, the difference was statistically significant improvement(P<0.05),but there are no statistically significant differences between minimally invasive group and open group in the terms of postoperative anterior vertebral height, Cobb angle, sagittal index(P≧0.05).During the follow-up period,the losses of correction of minimally invasive group and open group had no significant statistically difference(P≧0.05).3ã€The evaluation of pain recoveryBoth of the two groups the pain symptom will be obviously relieved.Compared with the preoperative level,the reduced degree of two degree has improved significantly, there was statistically significant difference(P<0.05).Minimally invasive group symptom of pain has been obviously improved, compared with the open group after 3 months,the differences have statistical significance(P<0.05), In minimally invasive group, the symptom of pain has been obviously improved, compared with the open group,there is statistical significance(P≧0.05).Conclusion:1.Under the precondition of master vitreoretinal surgery in the treatment of thoracolumbar fractures, minimally invasive and open surgery treatment effect are equivalent.2.Although the " X-ray " exposed of minimally invasive surgery process more time, but in the operation time, intraoperative blood loss, postoperative operation area drainage volume, length of hospital stay, and complications and postoperative recovery of curative effect is superior to open surgery. |
